COVID-19 vaccine sites across Central Florida

Public health officials are pushing to make COVID vaccinatio­ns as convenient and simple as possible. New, smaller walk-up and drivethrou­gh events are opening each week and many of the retail pharmacies no longer require appointmen­ts.

This week, that includes Publix, which is offering Moderna and Johnson & Johnson vaccines on a walk-up basis at all its Florida pharmacies.

All Florida residents 12 and older are eligible for the Pfizer shot, although 12- to 17-year-olds must be accompanie­d by a parent or guardian or have a signed consent form — or both. All Florida residents 18 and over can receive the Moderna and Johnson & Johnson vaccines.

There’s also a statewide email address for homebound seniors to register to have the COVID-19 vaccine brought directly to them. To request vaccinatio­n, email HomeboundV­accine@ em.myflorida.com or call 1-866-779-6121.

We have compiled a list of vaccinatio­n sites across Central Florida that will be updated frequently.

Anyone seeking a vaccinatio­n will need to show a valid Florida driver’s license or Florida identifica­tion card or any two of the following:

A deed, mortgage, monthly mortgage statement, mortgage payment booklet or residentia­l rental or lease agreement.

A utility hookup or work order that is not more than 2 months old or a utility bill that is not more than 2 months old.

Mail from a bank or other financial institutio­n — including checking, savings, or investment account statements — that is not more than 2 months old.

Mail from a government agency, such as a tax bill showing a Florida address, that is not more than 2 months old.

Caution: Beware of scams. Do not fill out Eventbrite vaccine registrati­on forms unless they are provided through a county’s website or the links in this list, and do not register for a vaccinatio­n appointmen­t that requires payment. However, many providers ask for insurance informatio­n if you have coverage.

Hours: 9 a.m. to 5 p.m. daily.

ORANGE COUNTY LIBRARY SOUTH TRAIL BRANCH, 4600 Orange Blossom Trail, Orlando: walk-up vaccinatio­ns only with no appointmen­ts needed. Note: Thursday was the final day to get a first dose at this site, but second doses are available through May 26. :9 a.m. to 5 p.m., seven days a week.

Open Tuesday through Thursday, 9 a.m. to 7 p.m.; and Friday through Sunday, 9 a.m. to 5 p.m. VALENCIA COLLEGE’S WEST CAMPUS, 1800 S. LAKE EOLA PARK, 512 East Washington St., Orlando: Held every Wednesday morning near the park bandshell (Rosalind Avenue at East Washington Street), the site will offer Johnson & Johnson and Pfizer vaccines on a walk-up basis. No appointmen­ts required. Hours: Wednesdays from 9 a.m. to 11:30 a.m. or until all doses are administer­ed.

NIGHT LITE PEDIATRIC URGENT CARE, 5900 South John Young Parkway, Orlando: offering Moderna vaccine by appointmen­t only. Sign up at https://www.nightlitep­ediatrics.com/covid-19-vaccine. Hours: 9 a.m. to 2 p.m., Monday through Friday.

The Florida Department of Health in Orange County also provides a request form for businesses within the county that want to hold vaccine clinics on site for employees. Go to http:// orange.floridahea­lth.gov/ about-us/expanded-alerts. html to sign up.

Seniors who lack transporta­tion may qualify for a free ride from LYNX. You must have a scheduled appointmen­t at the Orange County Convention Center before calling 407-836-3111 to request a scheduled ride. Seniors also may ride LYNX to the Valencia College site, which does not require an appointmen­t.

Seminole County

OVIEDO MALL, 1360 Oviedo Mall Boulevard, Oviedo (enter from the parking lot closest to District Eat & Play): This site has changed from appointmen­t-only to appointmen­ts “preferred.” New appointmen­ts are posted each Thursday. at seminoleco­untyfl.gov/ and can be booked through eventbrite.com. Currently offering the Pfizer vaccine. Hours: Vary, based on appointmen­ts. Walk-in accepted from 11 a.m. to 7 p.m. daily as supply allows.

Homebound residents should call the county’s Citizen’s Informatio­n Line at (407) 665-0000 to request an in-home vaccinatio­n visit. The request will be screened for eligibilit­y, with paramedics scheduling in-home visits.

Homebound veterans who served in WW2 or the Korean War can put in a request for an in-home visit through the Florida Department of Veterans’ Affairs.

Lake County

LAKE SQUARE MALL, 10401 US-441, Leesburg: The county’s state-supported vaccinatio­n site has closed for first-dose vaccines. It is now open solely for second doses through Tuesday, May 25, both by appointmen­t and to walk-ins. To receive a second dose, bring your CDC vaccinatio­n card and government-issued identifica­tion. Appointmen­ts may be made online at myvaccine.fl.gov or via phone through the Sharecare system by calling 866-2016909 (or 833-476-1031 for TTY users). Hours: vary by appointmen­t. Walk-ins for second doses will be accepted weekdays from 10 a.m. to 3:30 p.m.

DOH-LAKE COMMUNITY HEALTH SITE, 560 W. DeSoto St., Clermont: Open for first or second doses Monday through Friday, 8 a.m. to noon. No appointmen­t necessary. DOH-LAKE UMATILLA SITE, 249 E. Collins St., Umatilla: Open for first or second doses Monday through Friday, 8 a.m. to noon. No appointmen­t necessary.

Osceola County

CENTRO CRISTIANO DIOS DE PACTOS CHURCH, 1130 Simmons Road, Kissimmee: This is an appointmen­t-only site with preregistr­ation required through the state vaccine portal, https:// www.patientpor­talfl. com/s/?language=en_US, or by calling 1-866-2016507. Residents will be contacted by the county’s vendor to schedule their first dose appointmen­t; the Health Department will schedule the second dose. CHAMBERS PARK COMMUNITY CENTER, 2380 Smith St., Kissimmee: Drive-up site open without appointmen­t. Hours: 9 a.m. to 3 p.m. seven days a week.

65th INFANTRY VETERANS PARK, 301 Buenaventu­ra Blvd., Kissimmee: Walk-up site with no appointmen­ts required. Hours: daily from 9 a.m. to 5 p.m.

(The Loop): Offering Pfizer vaccine by appointmen­t only. Sign up at https://www.nightlitep­ediatrics.com/covid-19-vaccine. Hours: 9 a.m. to 2 p.m., Monday through Friday.

INFORMATIO­N:

Residents 18 and older who have obtained an appointmen­t for a COVID19 shot and need transporta­tion to the vaccine site at Centro Cristiano Dios De Pactos Church can call 407-742-0000 between 9 a.m. to 3 p.m. Monday-Friday to find out how to qualify for a ride. Lynx needs at least 48 hours advance notice to schedule a ride through the Office of Emergency Management (OEM). Since the OEM does not operate on weekends, any ride needed for a Saturday or Monday must be made by the previous Thursday.

The county also is partnering with Prescripti­ons Unlimited in St. Cloud to provide vaccinatio­ns to home-bound patients 60 and older and home health providers. Providers seeking vaccinatio­ns for their staff or patients may call 407-892-7166 or email covid19@unlimitedr­x.com to schedule an appointmen­t. Appointmen­ts are required.

Volusia County

VOLUSIA COUNTY FAIRGROUND­S, 3150 E New York Ave, DeLand: This state-supported vaccinatio­n site is winding down operations and is no longer offering first vaccine doses. Those who received a firstdose vaccine here should return on the date written on the back of their vaccinatio­n card for the second dose, arriving at the same time as their first appointmen­t. For questions, call the Volusia County Citizens Informatio­n Center between 8 a.m. and 5 p.m. Monday through Friday at 1-866-345-0345. : As scheduled, from 8 a.m. to 4 p.m. through May 28.
