Pea Ridge High School 781 W. Pickens Road, Pea Ridge

Priority violations: Maintain salad bar at 41 degrees or below or use time as a control; ice will only work if it completely surrounds the food items. Core violations: Condensati­on leak in freezer; do not store open items below leaks until repaired; check to ensure seals are working properly. Floor drain in dish room is higher than the floor so the water does not drain properly; repair by 2013 fall school year. Base cover is coming unglued in dish room.

Priority violations: Expired packages of lunch makers and cooked ham in customer refrigerat­or. Core violations: Box of oranges stored on the floor. Bags of charcoal on bags of sugar. Gnats around vegetable bin.

E-Z Mart 411 137 S. Curtis Ave., Pea Ridge

Priority violations: Coffee Mate creamer expired. Core violations: Need soap for handwashin­g, not just in the bathroom. Scoop stored on wooden shelf; store scoop on or in something that can be cleaned regularly and is nonabsorbe­nt. Replace weather stripping on back door. Mop sink is supposed to be used for mop water; do not dump mop water outside; sign over sink says to do so.
