Second facility closes

State bars assisted living home from taking in new clients after a resident died April 7

An assisted living facility in Dania Beach has been barred from accepting new residents after two staffers failed to provide medical care to a resident who died after falling in the bathroom.

The April 7 incident occurred just one day after the resident checked intoWillow Manor Retirement Living, a 165-bed facility at 150 Stirling Road.

The state’s Agency for Health Care Administra­tion issued an emergency order on April 25 barringWil­lowManor fromtaking in new clients.

According to state records, a nursing assistant atWillow Manor and saw blood pooling head.

The assistant summoned a nurse, who called 911 but failed aid.

The residentwa­s transporte­d to the hospital by paramedics and pronounced dead.

Officials at Willow Manor could not be reached for comment despite three calls to the center.

The facility was already in trouble with the state after several residents with dementia suffered mysterious injuries that were never investigat­ed by the facility, state records show.

In one incident, an emergency roomdoctor questioned the staff’s claim that a resident’s injuries were caused by a fall, noting the existence ofwounds on both sides of the body.

Willow Manor agreed witnessed the fall around the resident’s last

registered to provide year to relinquish its license in mid-2016; the state agreed it could stay open through June 30, 2016, to allow time for the relocation of residents.

It is the second assisted living facility in Broward County to come under a state crackdown in recentweek­s.

CoolidgePa­lms inHollywoo­dwas forced to close in April after an aide hit an 80-year-old woman in the head with a water pitcher several times, drawing blood and forcing a visit to the hospital for stitches.

The aide at Coolidge Palms told aHollywood police officer who responded to the Jan. 26 incident that thewoman had bit her arm.

But a state investigat­or who watched a surveillan­ce video of the incident said the resident never bit the aide.
