Hazel Marie Chopko – July 18, 1932 – November 2, 2021 Peoria, AZ - Heaven has gained another angel. Hazel was born in Durbin, WV and was one of 13 brothers and sisters. At the age of 20 she married her first husband Amos Puffenbarg­er and relocated to Phoenix, AZ in 1963. Her first job in Phoenix was at the famous Western Village Hotel as a Front Desk Clerk. She later went to work for the Arizona State Credit Union from which she retired in 1994 after 30 years of service. Hazel later married Raymond Chopko Sr. in July of 1977. They were married for 28 years before Ray’s passing in November of 2005. Hazel loved all animals, but in particular all of her dogs throughout the years. She was a big fan of the Arizona Cardinals and Diamondbac­ks and was lucky enough to be at the World Series final game in 2001. Hazel loved everybody and was always giving hugs to everyone and making sure she told you she loved you. Those hugs will be greatly missed. She is preceded in death by her husband Raymond Chopko Sr., several brothers and sisters and her first husband Amos Puffenbarg­er. She is survived by her daughter Bonita “Bonnie” Carrillo and her husband Vern; her stepson Ray Chopko Jr. and his wife Nancy along with their children, Natalie, Brandon, Nicole and Camille; as well as her sisters, Wilma, Wanda Kay, Dolores, Frances and Joann. She also leaves behind her precious dog Sassy. An 11 am Visitation will be held on Tuesday, November 23, 2021 followed by a Service to honor her life at 12 pm at Heritage Funeral Chapel 6830 W. Thunderbir­d Rd. Peoria, AZ 85381. A Committal Service at National Memorial Cemetery of AZ will follow at 2 pm. Donations to honor Hazel may be made to your local Alzheimers/Dementia Organizati­on or your favorite No-kill Shelter.
