We included only three major threats to patient safety: a suicide or significant attempt; a report of unwanted sexual contact; or a serious physical attack. The listings do not reflect other kinds of violations, such as problems with medication or sanitation. Unless otherwise noted, the safety incidents come from federal inspection reports from 2011 through the third quarter of 2017. Some of these hospitals have not been open for that entire period. The bed count comes from state data.
