RESTAURANT INSPECTIONS
Here are the food facilities that inspectors temporarily shut down because of imminent health hazards between April 29and May 6 in Riverside and San Bernardino counties. If no reopening date is mentioned, the facility had not been listed as reopened as of Friday, May 6.
Restaurant: Jack in the Box, 29993 Canyon Hills Road, Lake Elsinore Closed: May 6
Grade: 81/B, failing Reason: Sewage backup. The inspector said sewage was backed up throughout the food-prep areas, and plumbers were on scene.
Restaurant: Tacos El Chamuco, food truck based at 3815 Wabash Drive, Jurupa Valley

Reason: No valid health permit; when it was last inspected in October, it had a permit that expired at the end of that month.
Restaurant: Logan’s Roadhouse, 13480 Base Line, Fontana

Grade: Not graded (most recent was 92/A on April 18)
Reason: Cockroach infestation. An inspector visited in response to a complaint that someone saw customers kill a roach in the dining room. Management told the inspector they were aware of the issue and pest control had been out earlier that day as well as a week earlier. The inspector saw two live juvenile roaches in the servers area, two live and two dying roaches under the dishwasher and two dead roaches in other locations. Reason: Sewage overflow. All of the restaurant’s floor sinks were overflowing, with water from one of them running into the walk-in cooler when the handwashing sink was drained. The manager said a plumber had come by and was supposed to be back later that day. The inspector also noted that the restaurant’s health permit had expired in February. This was the restaurant’s second consecutive B grade, and it was temporarily shut down last April because of a cockroach infestation.

Reason: Rodent infestation. The inspector found rodent droppings at multiple locations, with a concentration by the fryer and under the bar refrigerators. Among other violations, there was also a heavy presence of fruit flies, especially around the bar floor sink, and house flies landing on food-contact surfaces in the kitchen; marinara sauce, pasta sauce, pizza sauce and ranch dressing weren’t being kept at safe temperatures; and equipment needed cleaning.

Reason: No hot water, nonfunctional oven hood. The inspector was there for a plan check consultation as part of a transfer of ownership. Reopened: The hot water was restored and the hood was fixed by May 2, and the facility was told it could reopen as soon as the transfer was approved.
Non-closure inspections of note
Here are selected inspections at facilities that weren’t closed but had other significant issues.
Ktrachos 504, at 2834 W. Rialto Ave. in Rialto, was inspected May 4 and received a grade of 76/C with one critical violation, for eggs not being kept cold. Among the 15 other violations, the inspector saw 10 old rodent droppings under the water heater (but no signs of an active infestation), and someone’s cellphone was on a food-prep board at the cook line. Co., at 5280 Arrow Highway in Montclair, was inspected May 4 in response to a complaint that 12 people got sick after eating there. It received a grade of 83/B with one critical violation. Two items at the salad bar, four at the pasta and soup station and the mix in the soft serve machine were at unsafe temperatures. Among the nine other violations, there was pink slime in the ice machine (not touching ice), two handwashing sinks didn’t have hot water, and two cans in storage had dented seams and broken hermetic seals, Magic Lamp Restaurant, at 8189 Foothill Blvd. in Rancho Cucamonga, was inspected May 4 and received a grade of 84/B with one critical violation. Some food was at unsafe temperatures in a walk-in refrigerator and a reachin refrigerator, neither of which was keeping cold enough. Among the eight other violations, there was mold in the ice machine (not touching ice). This was the restaurant’s fourth B or C grade since 2018.
Ahumada’s Market, at 7614 Evans St. in Riverside, was inspected May 3 and received a failing grade of 82/B with one critical violation. Some of the floor drains weren’t draining properly. Among the 11 other violations, there were rodent droppings in a nonfood storage area and a few flies and gnats in the restaurant, salsa was at an unsafe temperature in a cooler that was impounded for not keeping cold, and numerous areas needed cleaning. The market was given 24 hours to get plumbing and pestcontrol service. This was the facility’s third failed inspection since 2019.
Kuma Sushi, at 1905 N. Campus Ave. Suites C & D in Upland, was inspected May 3 and received a grade of 84/B. Among the 11 violations, several containers of food at the sushi bar were at unsafe temperatures, and the inspector saw 15 dead cockroaches on the floor under kitchen equipment. The inspector went back the next day and saw no signs of roaches.
The Jersey Mike’s Subs at 27614 Clinton Keith Road Suite 101 in Murrieta was inspected May 3 and received a failing grade of 85/B with one critical violation. Thirteen blocks of meat and cheese were at unsafe temperatures in the deli display case, which was impounded for not keeping cold. Among the eight other violations, the deli case was also covered in old food residue and debris, as well as a buildup of ice that was beginning to melt, putting the uncovered food inside at risk of contamination; and there was black mold in the soda machine ice chute. The Waba Grill, at 1594 W. Base Line in San Bernardino was inspected May 2 and received a grade of 77/C with two critical violations. Two workers didn’t wash hands, and some chicken and cooked carrots weren’t being kept hot enough. Among the 12 other violations, there were two dead roaches in a storage area (but no signs of an active infestation) and someone’s cellphone was on the foodprep table. A follow-up inspection was planned to check for roaches again. The Mr. You Express at 12223 Highland Ave. Suite 112 in Rancho Cucamonga, which received a 70/C on April 13, underwent a mandatory rescore inspection May 2. This time it got an 82/B, with one critical violation. There was mold and slime in the ice machine, with condensation dripping over it into the ice. Among the 11 other violations, chicken wasn’t being kept warm enough and some areas still needed cleaning. This was the restaurant’s sixth B or C grade since 2017.
Sushi Martini, at 8153 Aspen Ave. in Rancho Cucamonga, was inspected April 29 in response to a complaint about unsanitary practices. It received a grade of 80/B with one critical violation. Multiple items of food were at unsafe temperatures in a cooler that wasn’t keeping cold. Among the eight other violations, the inspector found eight dry rodent droppings on a shelf in a chemical storage area, five near the soda storage area and three on top of the dishwashing machine; the floors were very dirty; there was mold in the ice machine (not touching ice); the cook didn’t know the cooking temperature for chicken or how to cool down food safely; and another employee didn’t know dishwashing procedures. Pest control had just been out a few days earlier and reported seeing no activity. A followup inspection was planned to make sure there was no rodent infestation.
Updates from past weeks
The gym and stadium concession stands at Citrus Hill High School, at 18150 Wood Road in Perris, which were closed March 17 for not having current health permits, were permitted to reopen May 5. Both were reinspected and received A grades.

Inspectors visit food facilities for routine inspections, follow-ups and complaint investigations. In graded inspections, the facility starts with a score of 100 and can lose between 1 and 4 points for any of about 50 health code violations. A grade of 90 or above is an A, 80-89 is a B and 79 or below is a C. Riverside County considers B and C grades to be failing; San Bernardino County does not have a pass/fail system.
