Priority foundation violations: Multiple flying insects were located in kitchen prep area. Core violations: Dirty wiping cloths were stored on food prep surfaces. Layer of dust and food residue on shelves in kitchen area. Food residue on walk-in cooler shelves. Coving was missing along bottom of floor around food prep area. Floor in kitchen was peeling and cracking in some areas, making the floor not smooth and easy to clean.

Core violations: Two cartons of personal farm fresh eggs were stored on a wire shelf that had covered containers of sliced cheese. A box of cups and a box of lids were stored on the floor in the storage area. There was a large amount of ice buildup in the walk-in freezer due to the door not sealing properly.

Priority violations: The sanitizer to the dishwashin­g machine was empty and was not hooked up correctly to the dish machine. Observed a black buildup of debris in the ice machine, food debris on the slicer and bar fruit caddy was stored in the bar ice.

Priority foundation violations: Carton of shell eggs was stored on a wire shelf over bags of ready-to-eat produce. Core violations: No documentat­ion of a certified Food Protection Manager. Drinks with no lids were stored in the food prep area. A cell phone was in the food prep area. An electronic device to the television was stored in the bar handwashin­g sink. Disposable gloves and to-go containers were stored on the floor.
