Heather Locklear was hospitalized on a 5150 psychiatric hold on Nov. 18 — someone reportedly called 911 because she was having a breakdown — and now her family “is gathering information needed to make a move for conservatorship,” a source tells Us. “It’s very serious.” The actress’ lawyer has turned to Britney Spears’ legal team for advice, according to the source: “Heather’s family feels absolutely helpless and like this could be the only way to save her life. She won’t stay in treatment and consistently checks out early.” The 57-yearold’s ex-husband Richie Sambora and daughter Ava are among those most concerned. The Bon Jovi guitarist, 59, had been traveling since before fires ravaged California but rushed home to spend Thanksgiving with Ava, 21. “The joy of seeing my daughter and my house still standing,” he tells Us, “nearly brought me to my knees.”

Heather also was hospitalized for a psychiatric evaluation in June. Days later, she was arrested (inset) on two counts of misdemeanor battery on a police officer and emergency personnel.
