Eligibility for Medicaid/ Health Tracks
North Dakota Health Tracks is a preventive health care program for newborns, children and young adults through age 20 who are enrolled in ND Medicaid. Health Tracks is made up of comprehensive periodic screenings and well child checkups that help prevent and identify health concerns before they become too complex and costly to treat.
Health Tracks in Barnes County is made available through Buffalo Bridges Human Service Zone and City-County Health District (CCHD). The screenings are completed by a Registered Nurse and Social Worker at CCHD. A child is eligible to receive a screening 7 times during the first year of their life. Screenings are also recommended at age 15 months, 18 months, 24 months, 30 months and annually between the ages of 3 and 20.
During the screening process, staff conducts a physical examination, developmental and/or mental health screenings, vision and hearing checks as needed, lab tests, lead screenings for children between the ages of 6 to 72 months and any needed immunizations.
A simple dental screening is also completed which may include a fluoride varnish treatment and an orthodontic assessment. Referrals are made to dentists and orthodontists as needed.
The Health Tracks team also provides an assessment of your child’s growth and development, nutrition, provides parental guidance and referrals to community services. The team will help schedule appointments for services and address transportation needs to recommended services.
