Vaccine providers update schedules
The Auglaize County Vaccine Providers will continue scheduling for the eligible groups in Phase 1B of Ohio’s vaccine rollout. Each Vaccine Provider will have a limited supply of the vaccine for the qualifying priority groups.
The following groups currently qualify for COVID-19 Vaccine. If you do not meet the below criteria, please do not call for an appointment at this time. Please continue to monitor the Vaccine Providers’ websites, social media pages, and local media for future updates. We urge everyone to be patient during this process.
* 65 years of age and older
* Individuals with a severe congenital, developmental or early onset disorder.
Qualifying Conditions: cerebral palsy; spina bifida; severe congenital heart disease requiring hospitalization within the past year; severe type 1 diabetes requiring hospitalization within the past year; inherited metabolic disorders including phenylketonuria; severe neurological disorders including epilepsy, hydrocephaly, and microcephaly; severe genetic disorders including Down Syndrome, fragile X syndrome, Prader-willi syndrome, Turner syndrome, and muscular dystrophy; severe lung disease, including asthma requiring hospitalization within the past year, and cystic fibrosis; sickle cell anemia; and alpha and beta thalassemia; and solid organ transplants.
* Vaccine recipients must be age 16 or older to be eligible for the Pfizer vaccine, and age 18 or older to be eligible for the Moderna vaccine.
* Any individuals with one of the conditions above AND an intellectual or developmental disability who have not yet received the vaccine should contact their local board of developmental disabilities for assistance scheduling an appointment.
* K-12 School Staff: Please speak with your school administrators for additional information on how to receive the vaccine.
Grand Lake Health System:
Please visit our website at www.grandlakehealth.org to complete the appointment request form or call 419-300-1132. If the current Phase 1B criteria are met, patients will receive a phone call to schedule their appointment. If you have completed the online form

and left a message, your information has been received, please do not complete any additional forms or leave additional messages.
The Grand Lake Health System would like to inform eligible recipients that they will be able to fill out an online form available via the health system website that puts the patient on a waiting list. Eligible patients will then be contacted by phone to schedule a vaccine appointment. The hospital intends to use the health system website, patient portal, and social media to provide information to the public regarding vaccine clinics. Patients are encouraged to enroll in the portal and follow the website and social media to stay abreast of activity occurring in the health system.
Kroger:
Kroger of St. Marys encourages their customers to visit www.kroger.com/ohiocovidvaccine or to call their COVID-19 vaccine helpline at 866-211-5320 to schedule an appointment. All COVID-19 vaccines will require an appointment via www.kroger.com/ ohiocovidvaccine or 866-211-5320 as vaccines are available.
Auglaize County Health Department:
At this time the Auglaize County Health Department’s COVID Vaccine clinics are fully booked until March 5th, 2021. This is in part due to the rescheduling of earlier clinics as a result of vaccine delivery delays. It is anticipated that we will begin scheduling again for our clinics at 8:30 am on March 4, for the week of March 8th. Schedule availability will depend on the amount of vaccine we receive.
