MEDICATION review company Ward Medica on Management has launched a new “world first model of care,” which aims to reduce the risks of harm by providing individual­s with ongoing personalis­ed medica on plans based on DNA analysis.

Dubbed “Thrive,” the high‐touch clinical pharmacy service is being debuted together with aged care providers Allity & Uni ng AgeWell.

“Thrive reduces the risks of harm caused by taking mul ple medica ons through DNA analysis, in‐depth collabora on with an individual’s full health care team, regular medica on plan updates and around‐the‐clock access to a team of clinical pharmacist­s specialisi­ng in geriatric medicine,” the company said.

A simple DNA swab is used to measure which medica ons an individual’s body can metabolise well, with the informa on feeding into the crea on of each pa ent’s personal medica on plan.

Ward MM CEO Fiona Rhody‐Nicoll said Thrive aimed to reduce the alarming number of drug‐related hospital admissions in Australia, and also highlights the cri cal importance of clinical pharmacist­s within the healthcare system.

The “precision medicine” ini a ve will roll out na onally from Jan 2019, and will be available from $52 per week for aged care home residents and $72 for those living independen­tly.
