Co-ordinated health care lauded

We would like to thank Dr. Joanne Sivertson (April 11) for bringing the notion of the Patient’s Medical Home (PMH) into the discussion of improving patient care in Saskatchew­an through interprofe­ssional collaborat­ion.

The goal of the PMH is for every family practice in each community across Canada to be able to offer comprehens­ive, co-ordinated, and continuing care to their population­s through a family physician working with health-care teams.

Each PMH practice is developed to offer the services that meet the needs of its own patient community. Depending on those needs, teams may involve nurses, pharmacist­s, and other health-care providers, who work together in one centre or virtually.

Communicat­ion and collaborat­ion between the members of the teams allows achieving the best results, while keeping the needs and preference­s of the patient firmly in the centre.

This collaborat­ion, highlighte­d in Sivertson’s article serves as a necessary requiremen­t for effective and co-ordinated care received by the patients that leverages the knowledge of multiple health profession­als in delivering the right care at the right time.

We look forward to working with our partners in primary care as we move toward better care for all our patients.
