Did the system fail?

RE: Revocation of Doctorate of Chris Spencer

An independent tribunal at University of Toronto has recommended that the 1996 U of T doctorate of Chris Spence, former director of education of TDSB, be revoked for plagiarism. Where was Mr. Spence’s thesis chair while he was writing his thesis? Where was his thesis committee when he was preparing his work? Were all these professors sound asleep? And where was the roomful of august professors who were supposed to be questioning him at the oral defence of his thesis? Is there a system failure at U of T? Or was this just an unfortunate fluke?
