The appointee shall report to the place as mentioned above within a period of twenty one (21) days from the date of issuance of this order failing which he shall forego his right to appointment and the same shall be deemed to have been cancelled ab-initio without any further notice.

II. Matriculation/Date of Birth Certificate.

III. Domicile certificate issued by the competent authority.

IV. Health Certificate from the concerned District Chief Medical Officer.

V. Valid relevant category certificate issued by tho competent authority, if any applicable.

VI. Any other relevant document/certificate as required under rules.
