Q I’m a 19- year- old. My left testicle is 10 times bigger than my right one and I’m suffering a lot because of this. Will it have any implicatio­ns on my sex life? And are there any non- surgical methods available for this problem?

You seem to be having Hydrocele, a condition in which there is an accumulati­on of fluid around the testis. It occurs, usually, due to an infection or an injury to the layers of tissues in the scrotum.

In India, it most commonly occurs due to Filarial infection which is spread by mosquitoes. If it is huge as mentioned by you then you need to undergo surgery to drain away the fluids. Medicines will not be useful. Hydrocele may affect your sex life due to mechanical hindrance. However, its effect is more pronounced on fertility as it can adversely affect the sperm quality.

Q I wanted to know the treatment for premature ejaculatio­n which happens if I don’t have sexual intercours­e for more a week. Is there any internal medicine or external ointment which could prolong my sexual drive?

It is common for men to ejaculate prematurel­y if there is a long gap between acts of sex. It will settle on its own in due course. However, if the condition becomes a regular feature then you need profession­al help. Premature ejaculatio­n can be managed either through sex therapy or through Pharmacoth­erapy ( treatment with medicines). In the former method the man, with the help of his wife, is trained to gain voluntary control over his ejaculator­y reflex. In the latter, the ejaculator­y reflex is slowed down or inhibited with medicines. But both these methods should be undertaken by specially trained profession­als to be effective and safe. So, find out a trained consultant in sexual medicine.
