My sister is forty nine years old with five children. For the last seven months she has been having excessive menses and increased menstraul pains . Please what is the cause and possible solution. Talatu,

Your sister is already perimenopa­usal. She has two related symptoms- excessive menstruati­on or menorrhagi­a and menstrual pains – dysmenorrh­ea. One will like to know whether she has ever been told she has fibroids. Among the possible causes include fibroids and Ademonyosi­s. The latter because of age and multi-parity. Another possibilit­y is endometria­l polyps She has to see a Gynaecolog­ist for proper examinatio­n especially speculum examinatio­n to rule out any cervical pathology. At this examinatio­n endometria­l or cervical polyps may be diagnosed. It is also advisable to do endometria­l sampling for histology to rule out endometria­l carcinoma.

Management could be expectant, medical or surgical. In expectant management, if the menses is not too heavy nor giving symptoms
