Your sister is already perimenopausal. She has two related symptoms- excessive menstruation or menorrhagia and menstrual pains – dysmenorrhea. One will like to know whether she has ever been told she has fibroids. Among the possible causes include fibroids and Ademonyosis. The latter because of age and multi-parity. Another possibility is endometrial polyps She has to see a Gynaecologist for proper examination especially speculum examination to rule out any cervical pathology. At this examination endometrial or cervical polyps may be diagnosed. It is also advisable to do endometrial sampling for histology to rule out endometrial carcinoma.
Management could be expectant, medical or surgical. In expectant management, if the menses is not too heavy nor giving symptoms
