NOTICE OF EXTRA-JUDICIAL SETTLEMENT OF THE ESTATE OF DECEASED NELIA GUTIERREZ DATU WITH WAIVER

Notice is hereby given that ALEXANDER R. DATU, MICHELLE D. SAMSON, MARICHELLE G. DATU, MARK ALEX G. DATU AND MARLON G. DATU, all of legal age, Filipino Citizens and resident of 171 J. Luna St., Dolores, Mabalacat City, Pampanga;

That they are the surviving husband and legitimate children/ heirs of the late NELIA GUTIERREZ DATU;

That the aforementioned deceased/ decedent died on November 10, 2017 at 171 J. Luna St., Dolores, Mabalacat City, Pampanga;

That the said deceased/ decedent died without leaving any will and that they are the only surviving heirs;

That the deceased/ decedent is a member of GSIS and at the time of her death left a insurance/ benefit from the Government Service Insurance System, City of San Fernando, Pampanga;

That to the best of their knowledge and information, the said deceased left no debts;

That they are being all of legal age and with full capacity to contract, hereby by these presents, agree to adjudicate, as they hereby adjudicate, the insurance/ benefits left by the deceased, NELIA GUTIERREZ DATU to her surviving husband ALEXANDER R. DATU, their father.

This Extra-Judicial Settlement with Waiver is entered in the notarial registry of Atty. Venancio Q. Rivera III, Notary Public, as per Doc. No. 151; Page No. 80; Book No. XXXVIII; Series of 2018.
