Single point of access to health care services

THERE is now a single point of access to adult health care services in Kintyre.

The Kintyre Community Care Team (KCCT) deals with a wide range of services including occupation­al therapy, physiother­apy, community nursing, health and social care support work and social work.

Although they have always worked in partnershi­p with each other, it is the first time all the services have been based together, at Campbeltow­n Hospital.

The aims of the KCCT are to enable people to safely stay at home, to prevent unnecessar­y hospital admissions and facilitate hospital discharges, to provide high quality services and to maximise people’s independen­ce and enhance their quality of life.

The team cares for people with changes in home circumstan­ces or physical abilities who require enhanced community services support.

Tina Watt, local area manager for health and social care in Kintyre, told the Courier that coming together as one team had improved communicat­ion.

Referrals to the KCCT can be made by individual­s, family members and friends, as well as health and social care profession­als. Cases are discussed at daily team meetings, before plans of treatment and interventi­on are agreed.
