Single point of access to health care services
THERE is now a single point of access to adult health care services in Kintyre.
The Kintyre Community Care Team (KCCT) deals with a wide range of services including occupational therapy, physiotherapy, community nursing, health and social care support work and social work.
Although they have always worked in partnership with each other, it is the first time all the services have been based together, at Campbeltown Hospital.
The aims of the KCCT are to enable people to safely stay at home, to prevent unnecessary hospital admissions and facilitate hospital discharges, to provide high quality services and to maximise people’s independence and enhance their quality of life.
The team cares for people with changes in home circumstances or physical abilities who require enhanced community services support.
Tina Watt, local area manager for health and social care in Kintyre, told the Courier that coming together as one team had improved communication.
Referrals to the KCCT can be made by individuals, family members and friends, as well as health and social care professionals. Cases are discussed at daily team meetings, before plans of treatment and intervention are agreed.
