Care navigators to help elderly stay at home for longer

Health officials in Leicestershire have created “Care Navigators” to help elderly and vulnerable patients stay in their homes. The Care Navigators make home visits on behalf of GPS to check if patients have any extra care needs and provide help like obtaining practical equipment or linking them to community groups. In one year, 1,494 vulnerable people were helped.
