Baby girl died after cancelled surgery
A baby girl died after a series of failings in her care while waiting for a heart operation that was delayed three times, an inquest has heard.
Six-month-old Iris Day, who was born with a heart defect, died on 2 December last year. Her death came just a week before scheduled surgery at Evelina London Children’s Hospital.
An inquest in Chelmsford heard three operations had been cancelled. One cancellation was due to a lack of beds and a second was due to another ill child being prioritised.
