Baby girl died af­ter can­celled surgery

A baby girl died af­ter a se­ries of fail­ings in her care while wait­ing for a heart op­er­a­tion that was de­layed three times, an in­quest has heard.

Six-month-old Iris Day, who was born with a heart de­fect, died on 2 De­cem­ber last year. Her death came just a week be­fore sched­uled surgery at Evelina Lon­don Chil­dren’s Hospi­tal.

An in­quest in Chelms­ford heard three oper­a­tions had been can­celled. One can­cel­la­tion was due to a lack of beds and a sec­ond was due to an­other ill child be­ing pri­ori­tised.
