Immediate Closure

This facility is being closed due to lack of hot water, lack of hand washing and lack of sanitation throughout the facility. Observed the hot water at the hand washing sink turned off due to a faucet leaking water at the time of inspection. Instructed the person in charge (PIC) that a hand-washing sink shall be equipped to provide water at a temperatur­e of at least 100° F through a mixing valve or combinatio­n faucet. A self-closing, slow-closing or metering faucet shall provide a flow of water of at least 15 seconds without the need to reactivate. Instructed the PIC to fax a copy of the complete work order of the repair to CHPD. Observed the reach-in cooler with a mold-like substance. Observed the hand-washing sink faucet leaking water. Observed food employee fail to immediatel­y clean hands and exposed portions of arms after engaging in activities that contaminat­e the hands such as returning to the food prep area after taking dirty dishes to the dishwasher area then wiping hands on apron and dawning on gloves to prepare ready-to-eat food. Violation corrected on site by informing/observing PIC advise food employee to properly clean hands before handling ready-toeat foods. Instructed the PIC to retrain employees on the requiremen­t of when to wash hands as outlined in Section 2-301.14 of the 2009 FDA Food Code. Thai Spice, 7441 Paseo del Norte NE (April 12)

Downgrade

Observed pans in walkin cooler without being covered at the time of inspection. Observed chicken at 75°, whole potatoes at 70°, shredded cabbage at 68° at time of inspection. Instructed the person in charge (PIC) to toss out all the food mentioned. Observed that the PIC was not aware of the five (5) reportable illnesses at time of inspection. Gave the PIC a copy of the "send sick employees home now" form. Observed several pots in the cooler without common name, date or time when the product was prepared. Educated the PIC on proper labeling of food to prevent serving expired food to the public. Observed chemicals stored on the floor underneath hand-washing sink in the make line next to pots and pans at time of inspection. Instructed the PIC to remove the chemicals from the area. Observed employees have no knowledge of cooling methods. Observed all area of kitchen cart and make line had debris at the time of inspection. Observed vents in walkin cooler fans, and floors with debris at time of inspection. Observed hole in the wall underneath dishwasher sink at time of inspection.
