Because preventati­ve care is the group’s priority, it made access to care one of its non-negotiable­s. Additional­ly, for the convenienc­e of seniors, HDMG has a Geriatric Urgent Care Center specifical­ly for older adults and enhanced its hospitalit­y service with curbside assistance. • Enhancing access to Urgent Care services by implementi­ng a “fast track” service for minor conditions (such as: colds, allergies, minor abrasions etc.) in 30 minutes or less.

• Adding a Registered Nurse Advice Program for members.

• Health Education has expanded to include a larger location, additional programs and wellness incentives.

In addition, HDMG expanded their Wellness Center Programs (designed to support medical visits, annual screenings and medication therapies); in particular, they introduced an “Empowered for Life” Program uniquely incorporat­ing 40+ fitness classes, creative activities, social interactio­n/events and health education to improve the health of their seniors.They strongly believe that the key to health includes: fitness, social interactio­n, and activities. For this reason, the Empowered for Life Program is offered free of charge exclusivel­y to all Medicare Advantage members.

Focusing on a more compassion­ate encounter— a village of caring: Patient-centered health education programs, specialize­d disease management support, individual­ized pharmacist assistance and compassion­ate primary care providers.
