Healthcare Concierge
Your personal Health Care Assistant (HCA) will help coordinate your care by working directly with your healthcare provider. They will assist with preventative/ plan of care, answer questions, offer referral assistance and help to schedule appointments. The HCA team works closely with our Clinical Services teams to provide excellent care focused on prevention and overall wellness.
Hospitality Services
Our Hospitality Team will provide concierge services to patients, families and visitors. Patients with mobility impairments may request assistance to their destination within the facilities and campus areas.
Senior Wellness Visits
A free comprehensive preventative examination! This complimentary visit will enhance your Primary Care Physicians healthcare plan for you. The visit includes an overall health evaluation, gift card incentives and more! You can complete your wellness visit virtually, by phone, or in person. HDMG has earned 5-Star Recognition!
Empowered for Life Program:
This program is designed to support medical visits, annual screenings and medication therapies. We offer our Medicare Advantage Plan Members lifestyle opportunities through fitness, activities and social interaction! Look for exciting updates in 2022!
Medicare Resource Center
The center was created to provide a complimentary service to help navigate Medicare options. These decisions can be complex when choosing a plan. At the center, you have the opportunity to speak with a qualified licensed Medicare insurance agent.
