Woonsocket school registration open
WOONSOCKET — The Woonsocket Education Department has announced its annual district registration. Students from K-through-8 will be allowed to register at this time. Students are eligible for kindergarten if they turn 5 years of age before Sept. 1, 2018.
The following documents are required to register your child for school:
1. Birth Certificate – original
2. Proof of Residency – lease agreement, rent receipt, mortgage statement, electric bill. No cell phone bills or credit card bills will serve as proof of residency.
3. Immunization records: diphtheria, tetanus, pertussis, polio, rubella, measles, mumps, chicken pox, and hep- atitis. (Please note that updated immunizations may be turned in during the summer if not completely up to date. Please bring what you have for registration).
4. Proof of lead screening and physical exam with height, weight, blood pressure and vision exam. Exam can be dated one year prior to school entry.
5. Social Security and Medicaid card if available.
After March 10, all students registering for admittance to local public schools will need to register at central registration at the McFee Administration Building, 108 High St.
