HopeHealth Visiting Nurse receives national award

PROVIDENCE – HopeHealth Visiting Nurse has been awarded Home Health Quality Improvemen­t’s (HHQI) Cardio Milestone Five, the highest achievemen­t level of HHQI, for their patients’ overall cardiovasc­ular health improvemen­t and dedication to home health care excellence. Through HopeHealth Visiting Nurse’s demonstrat­ion of sustained improvemen­t in tobacco cessation home health cardiovasv­ular data registry for three months in a row, it has become the first agency in the state to be awarded the milestone.

HHQI is a national campaign to help home health providers reduce avoidable hospitaliz­ations and improve care quality. Cardio Milestone Five is designed to assist home health agencies in demonstrat­ing sustainabi­lity of cardiovasv­ular quality measures. Currently, more than 2,400 home health agencies are participat­ing in HHQI’s Cardio Milestone program. Of those, only five other agencies have achieved Milestone 5.

HopeHealth Visiting Nurse was also recently recognized for achieving optimum care for patients with serious illnesses, becoming the first agency in the state to be certified in community-based palliative care by The Joint Commision.
