HopeHealth Visiting Nurse receives national award
PROVIDENCE – HopeHealth Visiting Nurse has been awarded Home Health Quality Improvement’s (HHQI) Cardio Milestone Five, the highest achievement level of HHQI, for their patients’ overall cardiovascular health improvement and dedication to home health care excellence. Through HopeHealth Visiting Nurse’s demonstration of sustained improvement in tobacco cessation home health cardiovasvular data registry for three months in a row, it has become the first agency in the state to be awarded the milestone.

HHQI is a national campaign to help home health providers reduce avoidable hospitalizations and improve care quality. Cardio Milestone Five is designed to assist home health agencies in demonstrating sustainability of cardiovasvular quality measures. Currently, more than 2,400 home health agencies are participating in HHQI’s Cardio Milestone program. Of those, only five other agencies have achieved Milestone 5.
HopeHealth Visiting Nurse was also recently recognized for achieving optimum care for patients with serious illnesses, becoming the first agency in the state to be certified in community-based palliative care by The Joint Commision.
