Area worker deaths

October 2013: A 34-year-old woman died at the Wrigley plant in Chattanoog­a when a forklift hit a ladder she was climbing and threw her into an 8-inch pipe. Wrigley was fined $8,575 by the Tennessee Occupation­al Safety and Health Administra­tion for two serious and seven nonserious violations. February 2015: A 54-year-old man was crushed when a destacking machine at the Wrigley plant suddenly started up and dumped a load of trays on him, crushing his midsection. TOSHA fined Wrigley $28,000 for 10 serious violations.

January 2016: A 35-year-old removing concrete for a new stairwell in the Maclellan Building when a 12-foot-long, 2-by-6 joist fell from above and crushed his head. His employer, Environmen­tal Constructi­on Services Inc.., was fined $14,825 or four serious and two nonserious violations

June 2016: A 27-year-old worker at the M&M Mars candy factory in Cleveland, Tenn., installing insulation on ductwork was pinned between the railing of a scissor lift and the upper mezzanine floor, crushing the worker’s chest and back. M&M Mars was fined $7,900 for one serious and eight nonserious violations.

January 2017: A 35-year-old felling trees on Signal Mountain was struck by a falling tree and knocked off a bluff, falling to his death. Big Woody’s Tree Service Inc. was fined $12,300 for two serious and three nonserious violations.
