Reasons included bottled water in the prep cooler being used by a food service employee in the grill prep area; raw seafood being prepared in a facility with no parasite destruction letter; two bags of sugar found stored in grill prep area uncovered and subject to contamination; Potentially hazardous food cold-held at greater than 41 degrees Fahrenheit; sanitizer solution too strong for food contact surfaces; food being cooled by non-approved method; observed person in charge attempting to cool cooked white rice on the countertop with no ice bath; observed extreme buildup of grease/debris in/under/ around all nonfood surfaces including underneath the grill area/storage area/ reach in cooler areas/under shelving/ under prep tables/under the dishwasher area; entire prep area needs a cleaning schedule/deep clean/sanitizing by the staff; garbage in receptacles not protected from insects and rodents; observed live flies/gnats in the food prep area; dumpster area was in disarray; observed food debris/food equipment/buckets/ items stored behind facility at the rear kitchen door attracting pests.
