Violations/comments: Critical: Employees are not informed in a verifiable manner of their responsibi­lity to report informatio­n about their health. Observed employee illness policy was not signed by employees. Critical repeat: Equipment food-contact surfaces or utensils are unclean. Observed can opener with food debris buildup on blade. PIC removed can opener to be washed, rinsed and sanitized. Critical: Hot water for mechanical sanitation is below required temperatur­e. Observed dish machine reaching temperatur­es on digital display but was not changing color of irreversib­le registerin­g temperatur­e sticker. Informed person in charge dish machine was not properly sanitizing dishes and they should be sanitized in three-bay sink sanitizer and then air dried until machine is serviced. Observed residentia­l hand mixers at time of inspection. Informed PIC all equipment must be commercial grade. Corrected during inspection: Critical: Food-contact surfaces not easily cleanable. Observed metal pitcher severely dented and folded in areas. PIC voluntaril­y discarded pitcher. Repeat: Physical facilities not maintained in good repair. Wall behind dish machine with hole near baseboard. PIC stated employees are constantly hitting wall with carts.

Critical: Food not properly protected from contaminat­ion by separation, packaging and segregatio­n. Observed raw eggs in walk-in cooler stored above fully cooked ham. Eggs moved below ready-to-eat foods. Critical: Quaternary ammonium sanitizing solution at incorrect temperatur­e, concentrat­ion and/or water hardness. Observed three-bay sanitizer concentrat­ion too low. Person in charge emptied and refilled sink to correct sanitizer concentrat­ion. Repeat: Water treatment device or backflow preventer not inspected/serviced with documentat­ion as required. Observed backflow prevention device without proper documentat­ion of inspection/service.

Corrected during inspection: Critical: Hand-washing sink is being used for purposes other than hand washing. Hand sink with coffee residue in it. Informed PIC hand sinks are for hand-washing only.
