Violations/comments: Critical: Employees are not informed in a verifiable manner of their responsibility to report information about their health. Observed employee illness policy was not signed by employees. Critical repeat: Equipment food-contact surfaces or utensils are unclean. Observed can opener with food debris buildup on blade. PIC removed can opener to be washed, rinsed and sanitized. Critical: Hot water for mechanical sanitation is below required temperature. Observed dish machine reaching temperatures on digital display but was not changing color of irreversible registering temperature sticker. Informed person in charge dish machine was not properly sanitizing dishes and they should be sanitized in three-bay sink sanitizer and then air dried until machine is serviced. Observed residential hand mixers at time of inspection. Informed PIC all equipment must be commercial grade. Corrected during inspection: Critical: Food-contact surfaces not easily cleanable. Observed metal pitcher severely dented and folded in areas. PIC voluntarily discarded pitcher. Repeat: Physical facilities not maintained in good repair. Wall behind dish machine with hole near baseboard. PIC stated employees are constantly hitting wall with carts.

Critical: Food not properly protected from contamination by separation, packaging and segregation. Observed raw eggs in walk-in cooler stored above fully cooked ham. Eggs moved below ready-to-eat foods. Critical: Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness. Observed three-bay sanitizer concentration too low. Person in charge emptied and refilled sink to correct sanitizer concentration. Repeat: Water treatment device or backflow preventer not inspected/serviced with documentation as required. Observed backflow prevention device without proper documentation of inspection/service.

Corrected during inspection: Critical: Hand-washing sink is being used for purposes other than hand washing. Hand sink with coffee residue in it. Informed PIC hand sinks are for hand-washing only.
