MONTGOMERY COUNTY HEALTH INSPECTION­S

The following are a sampling of food operation inspection reports from the Public Health – Dayton & Montgomery County, from the week of Aug. 7 to Aug. 11. View full inspection­s reports at inspection­s.phdmc.org.

The 1 Eleven Flavor House

111 W. First St., Suite 110, Dayton

Date of inspection: Aug. 9 Violations: Observed wiping cloths on cooler top in bar area being held out of sanitizer. PIC moved cloths in sanitizer bucket at time of inspection.

Observed fish being held in water to thaw at time of inspection. Observed sliced tomatoes, pickles and lettuce held on countertop without proper time stamps. PIC added time stamps at time of the inspection and will change or discard after 4 hours. Comments: Reviewed time and lieu time stamping with PIC. -Spoke to PIC about date marking anything kept in facility over 24 hours.

-Observed sanitizer in 3 compartmen­t sink and bar area at proper concentrat­ion.

Dublin Pub

300 Wayne Ave., Dayton

Date of inspection: Aug. 8 Violations: Observed house made sauces, biscuits and gravy and ex. stored in walk-in and prep coolers without labels present on storage containers. PIC added labels at the time of inspection.

Observed coleslaw and sliced tomatoes on prep line holding at 45F at the time of inspection. PIC will place back in cooler or cover with lids to help maintain temperatur­e.

Observed house made sauces and food prepped in facility without date marking present. PIC added dates to containers at the time of inspection. Observed salad prep cooler stored next to dry storage room with standing water at the bottom of the unit. Per PIC the condenser is broken, and they are working on repairing. Comments: Spoke to PIC about obtaining correct test strips at time of inspection. -Spoke to PIC in regard to complaint on mixing chemicals in mop bucket and kitchen not closing as a response. Per PIC dishwasher mixed chemicals at the end of the night and the kitchen was already closed for the night mop bucket was emptied and kitchen was cleaned after. Spoke to PIC about ensuring chemicals are not mixed and used according to directions.

Leo’s Mexican Market

748 Troy St., Dayton

Date of inspection: Aug. 10 Violations: Chicken in the beer walk in refrigerat­or at 46 degrees F during the inspection. Needs to be 41 degrees F or below. Moved to the kitchen walk in refrigerat­or during the inspection that was cold holding foods below 41 degrees F and corrected.

2-ounce packages of fried pasta do not have full list of ingredient­s. Ensure that when 2 or more ingredient­s in a packaged food product for self-service that an ingredient statement is available. Discussed with the person in charge.

The chlorine level for the meat department sanitizer in sink at less than 50ppm. Ensure between 50ppm and 100ppm. Added chlorine and corrected. Quaternary ammonia at the kitchen area less than 150ppm during the inspection. Added more sanitizer and corrected. In the meat department the hand sink is blocked by the band saw. Ensure that the hand sink is accessible at all times. Discussed with the person in charge and moving the band saw. Several spray bottles of cleaners not labelled for contents. Ensure contents known and used properly. Labelled spray bottles during the inspection and corrected.

Comments: No mop holder at the mop sink.

General food walk in freezer emergency release mechanism not working.

Pest spray in the kitchen wait staff area.
