4021 Dayton-Xenia Road, Beavercree­k

Date of inspection: July 20 Violations: Pizzas found on the hot plate at temperatur­es between 117F and 135F. All pizzas are over 135F after coming out of the oven before being placed on the hot plate. When ordered, individual slices are reheated in the pizza oven to 165F. Any pizzas left at the end of the lunch period will be discarded. Found staff conducting the procedure as posted.

Found the two hand sinks in the kitchen were not reaching 100F as required for proper hand-washing. Discussed with owner, who will adjust the water heater to increase the temperatur­e.

Found the can opener blade soiled. Discussed and PIC removed it to be properly cleaned. Found a container of salami stored past the seven-day use by date of July 19, 23. Found several baggies of TCS foods where the date marking was not legible. PIC discarded the items to correct. Found the wash water was at

100F. The discussion with the owner about turning up the water heater should also correct this violation. Wash water shall be 110F. Found kitchen floor tile/grouting in need of detailed cleaning. Found piping under the three-compartmen­t sink and under the hand sink in need of cleaning.

Found dust accumulati­on and isolated grease buildup in the back wall of the pizza oven. PIC stated the hood is to be cleaned next month and staff will keep up on the accumulati­on of dust. Comments: PIC was verbally informed of all violations noted during inspection. All signatures and initials on the inspection report have been digitally signed and agreed upon by both the PIC and the inspector. Inspection report will be emailed to PIC.

Date of inspection: July 18 Violations: Found no paper towels in the rear paper towel dispenser. PIC corrected.

Found no thermomete­r in the rear reach in freezer. Thermomete­r to be placed inside freezer to monitor internal temperatur­es. Found facility is self-treating for mice by placing sticky pads underneath sinks and in areas in back prep areas. Found no mice on sticky pads at time of inspection. PIC will remove sticky pads and contact a licensed PCO to have profession­al pest services provided to facility. Copy of invoice for pest treatment/prevention to be emailed to inspector once completed.

Found single-use to-go boxes and items being stored with the food contact surfaces unprotecte­d. PIC corrected.

Cleaning frequency and restrictio­ns:

1. Found the mop sink area to be dirty.

2. Found the floor in the back area by the back door to be dirty.

3. Found only 8 ft candles at the front-hand sink. To be replaced with lumens to reach a minimum of 20 ft candles.

Comments: Please have all violations corrected prior to next standard inspection.
