Q:

Two days ago, I started to feel a bit tender in my private area and found three “pimples” on my vulva. I squeezed one and something white oozed out. I tried to squeeze the others, but they wouldn’t budge. What are they, and what can I do to get rid of them?

A:

Those “pimples” sound like sebaceous cysts, which form when pores get blocked and sebum—the cheesy substance you describe— builds up beneath skin.

Sebaceous cysts typically resolve on their own, and squeezing isn’t advisable since it can lead to an infection. That said, there are steps you can take. I suggest applying a warm compress to the area for 10 to 15 minutes twice daily, or taking warm sitz baths for the same amount of time—simply soak in a bathtub filled with enough water to cover your hips. This will ease the discomfort and may help the cysts rupture and drain on their own. And to speed healing and prevent infection in the cyst that was squeezed, I recommend smoothing on a thin layer of over-the-counter antibiotic ointment.

Finally, since friction and sweat can encourage the formation of new cysts and aggravate existing ones, I suggest avoiding tight undergarments and wearing panties made of breathable materials such as cotton or silk. Also smart: opting for loose-fitting shorts or yoga pants rather than leggings when working out, then showering and changing into fresh underwear as soon as possible afterward.

These steps should bring relief within a week and help keep future problems at bay. But if soreness persists or worsens or the cysts enlarge to the point that they interfere with normal activities (including sex), see your doctor. She can remove them with a simple in-office procedure and advise you on other prevention strategies.
