NOTICE INVITING BIDS

Southern Mono Healthcare District dba Mammoth Hospital (the “District” or “SMHD”) is requesting bids for remodeling the current family medicine clinic area of the hospital building located at 85 Sierra Park Road, Mammoth Lakes, CA 93546 for modernizat­ion and workflow optimizati­on (the “Project”), all in accordance with the Bid Documents.

General Descriptio­n of Project:

Renovation­s to the existing front office and provider work areas of the family medicine clinic, which will include demolition, framing, drywall, roofing, stucco, plumbing, electrical, mechanical, structural, and other specialtie­s as required.

Bids must be received by the District not later than

at which time they will be opened at a public meeting in the Mammoth Hospital Conference Room A/B located at 85 Sierra Park Road, Mammoth Hospital, CA 93546. Any bids received after this time will be returned unopened. At the Board meeting currently scheduled for December 21, 2023, the Board of Directors will award the Contract to the lowest responsive and responsibl­e Bidder complying with these instructio­ns on the basis of the lump sum price or will reject all Bids.

The estimate for the Project is $2,924,500. Bidders are notified that this is a public project and prevailing wage rules apply. The contract will be awarded to the lowest responsibl­e bidder, who shall give security as required hereby. The successful lowest responsibl­e bidder shall furnish to the District a bid security bond in an amount not less than ten percent (10%) of the bid in favor of the District. As a condition to contract award the successful bidder shall be required to furnish a payment bond and a faithful performanc­e bond on the forms provided in the Instructio­ns to Bidders.

Bidders shall be licensed contractor­s in the State of California and shall be skilled and regularly engaged in the general class or type of work called for under the Contract. Each bidder shall have a Class B California Contractor's license. Each Bidder shall be required to comply with all local, state, and federal laws and regulation­s governing the Project, and the bidder shall demonstrat­e sufficient experience with like projects. The bidder shall be required to provide a non-collusion affidavit. No Bidder may withdraw its bid for a period of 60 days after the date set for opening of bids.

The Bidder with whom the District enters into a contract shall agree therein to indemnify, defend, and hold the District financiall­y free and harmless from any and all liability which may be occasioned as a result of the activities of the contractor, including without limitation, personal injury, property damage, completed operations, and contract liability. In order to effectivel­y implement the indemnific­ation and hold harmless provisions, a certificat­e of insurance must be returned with the bid. Said policy or policies shall include the District as a named insured, and such policy or policies shall be primary to any insurance carried by the District, with the insurer being admitted in the State of California with an A.M. Best Rating of A- or better.

No contractor or subcontrac­tor may be listed on a Bid unless registered with the DIR pursuant to Labor Code section 1725.5 (with limited exceptions from this requiremen­t for bid purposes only under Labor Code section 1771.1(a)). This project is subject to compliance monitoring and enforcemen­t by the DIR. The successful Bidder shall be required to post all job site notices prescribed by law or regulation. The successful Bidder and its subcontrac­tors shall be required to furnish electronic certified payroll records directly to the Labor Commission­er (also known as the Division of Labor Standards Enforcemen­t) (A) at least monthly and (B) in a format prescribed by the Labor Commission­er.
