Montefiore: providing comprehensive care to the Bronx and beyond
Montefiore Health System’s successful network of primary care practices, known as The Montefiore Medical Group (MMG), has been providing exceptional healthcare services to families since 1947. Both its primary care doctors and specialists treat the unique needs of children and adults at more than 20 convenient locations throughout the Bronx and Westchester County. MMG is dedicated to supporting and understanding the unique needs of communities where Montefiore has a presence. The primary care network has played an instrumental role in Montefiore’s long-standing mission to provide a fully cohesive healthcare delivery system. In addition, MMG functions as an important training venue where the physicians of tomorrow learn to care for patients in one of the most sophisticated urban primary care environments available anywhere.
This May marks the 50th anniversary of two of MMG’s most established sites: The Comprehensive Health Care Center (CHCC) in the South Bronx, and The Comprehensive Family Care Center (CFCC) in the East Bronx. During their 50-year history, both practices have consistently innovated to provide excellent interdisciplinary team-based behavioral health and primary care to large populations with complex clinical and psychosocial needs. This approach to primary care has resulted in sustained performance at the highest level of excellence exemplified by rates of pediatric immunization, diabetes treatment, depression screening, mammography and cervical cancer screening, infant developmental evaluation, and tobacco cessation interventions that are among the highest in the region.
Many MMG sites, including CHCC and CFCC, are members of the Bronx Community Health Network (BCHN), a Federally Qualified Health Center (FQHC). BCHN-sponsored FQHCs provide access to affordable, quality, comprehensive health care that includes dental, mental health, social work, nutrition, health education and pharmacy services. A team of healthcare professionals work together at each clinical practice to offer an integrated system of care that ensures collaboration among all of a patient’s medical providers. No one is refused care because of inability to pay.
What was built in the Bronx more than a century ago, has now expanded to become one of New York’s premier academic health systems and a recognized leader in providing exceptional quality and personalized, accountable care to approximately three million people in communities across in the Bronx, Westchester and the Hudson Valley. Montefiore Health System is home to 11 hospitals, including the Children’s Hospital at Montefiore, Burke Rehabilitation Hospital and close to 200 outpatient care sites. The advanced clinical and translational research at our medical school, Albert Einstein College of Medicine, directly informs patient care and improves outcomes.
