Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Noncritical violations: No thermometer in three-door refrigerator (on order). Ice scoop was stored on top of a box, store scoop in or on something that can be cleaned and sanitized. Facility has test strips for quaternary ammonia, not bleach, and there is no quaternary ammonia on location.

Pizza Hut

391 U.S. 412 East, Siloam Springs Critical violations: Employee did not wash hands between dirty dishes and handling clean dishes. Employee went to food preparation and did not wash hands first.

Noncritical violations: No effective hair restraint worn by manager during food preparation. Jewelry on wrist of food employee. Clean prep table door and door frame; edges of salad bar white board have a buildup on them; clean upright freezer gaskets and door. Sewer gas smell in back area by dish room; check to see if grease trap needs to be pumped out. Posted permit is expired.

Noncritical violations: Air conditioner unit is out in building so doors are open, keep flies under control; unit will not be repaired until Monday. Excessive water/ice buildup on the floor in the freezer walk-in; new unit should not have a condensation leak; work order submitted already.

Critical violations: Dishwasher is not getting the plates hot enough to sanitize; gauge for wash cycle is reading 140 degrees and should be at least 160 degrees, final rinse cycle does not always reach 180 degrees on gauge; heat sensitive strip run through dish washer and did not change color as needed. Manually wash and sanitize until repaired (repeat violation). Food items checked in walk-in cooler are not at 41 degrees or below. Monitor to ensure unit is working properly, may have been in defrost mode; contact the Department of Health when repaired.

Noncritical violations: Backup thermometer is not accurate, reading below 30 degrees, but food checked is at 41-42 degrees; replace thermometer (repeat violation). Dishwasher does not maintain wash or rinse temperatures through multiple cycles as it should; contact a professional company or repair person to repair the machine, or replace if needed; this issue has been on going and needs to be corrected properly and permanently. Condensation leak in the walk-in freezer.

Walk-in is not holding food at 41 degrees or below; service call made during inspection (repeat violation).

Noncritical violations: Hot water faucet at three-compartment sink won’t shut off completely; excessive water condensation in both food walkin and beer walk-in; floor in beer cooler is no longer in good condition or easily cleanable; floor is no longer sealed so water is being absorbed into flooring material, replace as needed.
