Prep table raw products stored next to ready-to-eat food; store in a manner to prevent cross contaminat­ion between raw and cooked food. Need sanitizer buckets or spray in food-prep areas; sushi, cook line, prep area next to walkin cooler. Cut fruit at 48 degrees cold bar, cheesecake­s. No date-marking of temperatur­e control for safety foods held more than 24 hours. No logs kept for sushi rice or raw buffet.

Critical violations: Employee did not wash hands after coming in from outside and prior to opening cooling units. Eggs are not maintainin­g 135 degrees or above; heating element was not centered and could possibly be the reason.

Noncritica­l violations: Countertop at the edge of the cupboard is broken and jagged; no longer in good condition (repeat violation), may be corrected during remodel. Permit is posted but expired.

Received a complaint from city of Fayettevil­le that the facility was dumping waste cooking oil down the storm drain. City wastewater workers and ADEQ water inspectors met for inspection. Two people from different businesses saw Pavilion employee dumping waste oil down storm drain; waste container is full, and there is evidence in the storm drain of grease being dumped. ADEQ will follow up under Clean Water Act.
