Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.
BENTON COUNTY May 25 Tokyo House
401 N. 46th St., Rogers Critical violations: Food employees should use only designated handwashing sinks for handwashing; employee washed gloved hands in the three-compartment sink. Provide an adequate supply of hand cleanser at each handwashing sink; there was no soap in the hand sink in the beverage area. No identification for raw oysters. Prep table raw products stored next to ready-to-eat food; store in a manner to prevent cross contamination between raw and cooked food. Need sanitizer buckets or spray in food-prep areas; sushi, cook line, prep area next to walkin cooler. Cut fruit at 48 degrees cold bar, cheesecakes. No date-marking of temperature control for safety foods held more than 24 hours. No logs kept for sushi rice or raw buffet.
Noncritical violations: Time/temperature control for safety foods shall be thawed using an approved method; beef thawing ambient temperature.
May 26 Wingate Inn

Critical violations: Employee did not wash hands after coming in from outside and prior to opening cooling units. Eggs are not maintaining 135 degrees or above; heating element was not centered and could possibly be the reason.
Noncritical violations: Countertop at the edge of the cupboard is broken and jagged; no longer in good condition (repeat violation), may be corrected during remodel. Permit is posted but expired.

Noncritical violations: Using dish soap to wash kale (used as a garnish); chemicals used to wash or peel raw, whole fruit and vegetables shall meet the requirements specified in the regulations. Grout between tiles on the floor in the dish room is missing; tiles are loose or missing on the serving line.

Raw beef was observed in the back kitchen uncovered; foods shall be protected to prevent contamination.
Noncritical violations: Raw beef was thawing in a pot in the sink; foods shall be thawed under running water. The kitchen staff is wearing a wiping towel on belt and using the towel to dry hands and a knife; hand towels shall be used only to wipe plated foods for service; wearing towels on a person shall be discontinued.

Pavement is stained with garbage and grease, going to a storm drain. Pressure wash or clean visible grease and garbage on the surface. Received a complaint from city of Fayetteville that the facility was dumping waste cooking oil down the storm drain. City wastewater workers and ADEQ water inspectors met for inspection. Two people from different businesses saw Pavilion employee dumping waste oil down storm drain; waste container is full, and there is evidence in the storm drain of grease being dumped. ADEQ will follow up under Clean Water Act.

Noncritical violations: The back door was opened, and flies were seen in the kitchen; the entrances to the kitchen shall be protected from pests and rodents; ensure all openings are closed. There are broken tiles all around the kitchen line and the walkin cooler; the floor surfaces shall be smooth and easily cleanable; new tiles shall be installed to be in compliance.

Noncritical violations: In the refrigerator unit on the step-down area the condensation pipe is dripping, leading to a pool of water; on the bottom shelf by the walk-in cooler there is water accumulation from spraying the floors; all water accumulation shall be cleaned up when the accumulation occurs.
