Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

BENTON COUNTY Feb. 12 Domino’s Pizza

5092 W. Northgate Road, Rogers Critical violations: Spray bottle of chemicals did not have a label.

Noncritical violations: The back room door lacks a door sweep, and there is a gap large enough to allow insects, rodents or other debris into the facility. The faucet and drain for the three-compartment sink are leaking and need repair. Employee food was stored above facility’s food.

Noncritical violations: Food workers must wear proper hair restraints. A restroom door was propped open.

Critical violations: No prepared sanitizer was on site at the time of inspection. Employee was wearing bracelets that were not a plain band.

Noncritical violations: Multiple employee drink cups did not have lids. There was an unlabeled squeeze bottle of water at the grill.

Feb. 14 Cold Stone Creamery

3301 Market St., Rogers Critical violations: None Noncritical violations: Food workers must wear proper hair restraints. Vent cover and refrigeration unit need cleaning, repeat violation. Tiles in front of the walk-in freezer are missing, repeat violation; there is a condensation leak in the freezer which is causing an accumulation of ice on the shelves and floor.

1610 S. Eighth St., Rogers Complaint inspection. Critical violations: Raw meat was stored above ready-to-eat foods in the walk-in cooler and the reach-in cooler. Food items were not date marked as needed.

Noncritical violations: Food containers in the walk-in cooler were not covered. Sanitizer mixing unit is leaking water and not dispensing sanitizer. Shelves in the walk-in cooler, the walkin cooler handle, the freezer handle and areas under and around the grill need cleaning.
