Critical violations are those factors leading to foodborne illness and must be corrected immediately. Noncritical violations relate to maintenance of food operations and cleanliness.

BENTON COUNTY July 10 Dharvi Plaza of Arkansas

Critical violations: No soap at hand sink in butcher area. Knives in hand sink.

Noncritical violations: Top of freezer in back storage area has dried liquid on it.

July 14 TNT Express

1196 Arkansas 59 South, Decatur Critical violations: Hush puppies 116 degrees front hot hold unit right side not keeping temperature. Butter, cheese and sauce (all shelf stable food), 45 degrees, in the walk-in cooler.

Noncritical violations: Fancy, the dog, occasionally wanders from the automotive area into the cafe area.

July 15 Dollar Tree

1443 U.S. 412 W, Siloam Springs Critical violations: Evidence of mice seen along the base board in the store room, inside the walk-in and around the bait/traps. Damaged packages seen on the shelf in the customer service area.

Noncritical violations: Hole in the wall on the left side of the mop sink. Posted retail food permit has expired.

Pittman, Prairie Grove Critical violations: Mayonnaise was observed at 47 degrees in the hamburger prep table and cheese was observed at 45 degrees in the small refrigerator under the hamburger prep table. Noncritical violations: A drink with a lid and straw and a Dr Pepper with a lid on were located over single serve paper products. Floor tiles in the kitchen are missing or cracked. Retail food permit was not posted.
