Restaurant inspections
Critical violations are those factors leading to foodborne illness and must be corrected immediately. Noncritical violations relate to maintenance of food operations and cleanliness.
BENTON COUNTY Dec. 7 CV’s Family Foods Deli-Bakery
200 First Ave. S.E., Gravette Critical violations: Deli counter and walk-in cooler found without date, for the third time. Employee had an open, uncovered coffee cup. Mashed potatoes, 125 degrees, in hot hold area and should be at 135 degrees or above. Green beans disposed of as no time control. Milk 17 days out of date, pumpkin pie filling 26 days out of date, both located in the deli walk-in fridge. Meat in the deli cold hold counter three days out of date.

129 Fowler St., Suite B, Gentry Critical violations: Uncovered pans of raw meat stored above a container of salsa. Salsa on ice at the servers station is not at 41 degrees or below. Food temperatures checked in the walk-in are not quite at 41 degrees or below. External thermometer is reading 36 degrees. The reason the food is not at 41 degrees or below is unknown.
Noncritical violations: Bags of onions stored on the floor in the store room. Container of beef on the floor in the walk-in cooler. Chip scoop at the server station is stored on a cloth towel.

2010 E. Main St., Siloam Springs Critical violations: Employees are not washing their hands between task changes. No paper towels at the handwashing sink. Salsa at the server station is not at 41 degrees or below. Noncritical violations: Food workers not wearing effective hair restraints. Grease collection unit has leaked all over the ground.

IHOP
4604 W. Walnut St., Rogers Critical violations: None Noncritical violations: Inspection was due to a complaint about servers not wearing masks during a weekend shift. All servers/hosts and kitchen staff were wearing masks at time of inspection.
Dec. 9 Fresh Fish Market Place
607 S.E. Fifth St., Bentonville Critical violations: Vegetables being rinsed in sushi bar handsink. Wall mounted potato slicer and can opener have accumulation of food debris. Noncritical violations: Cardboard boxes of food items being stored directly on floor of walk-in freezer. Posted retail food permit has expired. Condiments present on tabletops.
Dec. 10 Doomsday Coffee and Roasterie
120 U.S. 412 East, Siloam Springs Critical violations: No handwashing sink installed at the front counter. Clean and sanitize the interior of the ice machine including the drop guard and where the ice is made prior to use. Noncritical violations: All equipment that will be used for food storage or preparation shall be cleaned and sanitized as needed prior to use. Unfinished wood around cut-out by the coffee barista and the under-the-counter refrigerator needs to be finished to make smooth, easily cleanable and nonabsorbent. No self-closing doors on restrooms. Hole in the bottom of the wall where the cold brew station will be located.
