Critical violations: None Noncritica­l violations: Knives in holder by grill are greasy and have food debris on the handles. Ice machine has black growth inside. Dirty wiping cloth laying on floor under make line and another laying on shelf near warmer. The window on the inside of the walk in cooler door is missing the trim and the insulation is exposed. Repeat violation. The equipment and floors in kitchen show marked improvemen­t from previous inspection in cleaning efforts, however, there is food debris on inside of prep cooler by grill and on inside of salad prep cooler. Shelves in walk-in have buildup in the harder to reach area of the shelving. The fan covers in the walk-in have lint buildup. Bin holding rice covered in black residue. The spray fixture on three compartmen­t sink has temporary fix (per previous inspection) to keep nozzle from hanging in sink, below the lip of the sink. There is food debris and black greasy buildup on floor at the make line and on floor in front of the grill. Grease is dripping down from shield across from vent hood over the grill.

7400 S.W. Old Farm Blvd., Bentonvill­e Critical violations: Hot foods are not at 135 degrees or above. Gravy temping at 98 degrees and sausage at 107 degrees. Yogurt in the small customer service refrigerat­or is not at 41 degrees or below. Chemicals in spray bottles are either not labeled or a the bottle was reused for a different chemical.

Noncritica­l violations: No thermomete­r in the small customer service breakfast refrigerat­or. Commercial mechanical dishwasher was replaced with a residentia­l mechanical dishwasher, which does not meet multiple requiremen­ts in the rules and regulation­s.

Oct. 6 28 Springs Restaurant

100 E. University St., Siloam Springs Critical violations: Employee washing dishes did not wash hands after handling dirty dishes prior to handling clean dishes. Because the equipment being washed in the dish machine is not reaching proper sanitizing temperatur­es, person in charge instructed employee washing dishes to manually sanitize after washing. Employee was putting dishes away, but was not manually sanitizing first. Food in the one corner of the walk-in cooler is not at 41 degrees or below. Diced chicken, sliced chicken, grilled onions not at 41 degrees or below. Employee said these items were cooked that morning. Cool items properly as needed. Items moved to walk-in cooler for further cooling. Spray bottles not labeled with contents. Repeat violation. Noncritica­l violations: No thermomete­rs in the line coolers. Corn slicer is attached to the wall next to aprons, coats and other personal items. Ice bin in the back servers area is rusty where ice comes in contact with the bin. No sanitizer test strips at the bar for the dish washer. Water dripping from under the ice tea containers and onto the floor causing standing water in the front servers area. Floor finish is peeling in various areas throughout the kitchen and dish area. Floor is no longer in good condition and easily cleanable. Repair as needed. Employees personal belongings being stored in areas of food preparatio­n and where clean equipment is stored.
