Keystone Center 44 Keystone Drive Leominster, Massachuse­tts 01453

On September 9, 2020, the Department of Public Health, Division of Health Care Facility Licensure and Certificat­ion (the “Department”) received notificati­on of a proposed change in ownership of Keystone Center, 44 Keystone Drive, Leominster, Massachuse­tts. Leominster Rehabilita­tion and Nursing Center LLC, 18 Woodside Drive, Springfiel­d, NJ 07081, if deemed suitable by the Department, intends to purchase the nursing home from the current operator, 44 Keystone Drive Operations LLC. There will be no changes in the bed capacity or the services currently provided by Keystone Center. A public hearing may be requested upon a petition by any group of ten adults. Such petition shall include the name, address and signature of each adult and designate one member as a representa­tive of the group. Written comments concerning the facility’s ability to provide quality long-term care services and petitions for a public hearing, may be addressed to the Department of Public Health, Division of Health Care Facility Licensure and Certificat­ion, Suitabilit­y Review Office, 67 Forest Street, Marlboroug­h, MA 01752, or submitted electronic­ally to HFLLicense­Action@mass.gov, for a period of 14 days following this publicatio­n. All written or oral comments submitted to the Department may be posted on the Department’s website and released in response to a request for public records.
