On September 9, 2020, the Department of Public Health, Division of Health Care Facility Licensure and Certification (the “Department”) received notification of a proposed change in ownership of Keystone Center, 44 Keystone Drive, Leominster, Massachusetts. Leominster Rehabilitation and Nursing Center LLC, 18 Woodside Drive, Springfield, NJ 07081, if deemed suitable by the Department, intends to purchase the nursing home from the current operator, 44 Keystone Drive Operations LLC. There will be no changes in the bed capacity or the services currently provided by Keystone Center. A public hearing may be requested upon a petition by any group of ten adults. Such petition shall include the name, address and signature of each adult and designate one member as a representative of the group. Written comments concerning the facility’s ability to provide quality long-term care services and petitions for a public hearing, may be addressed to the Department of Public Health, Division of Health Care Facility Licensure and Certification, Suitability Review Office, 67 Forest Street, Marlborough, MA 01752, or submitted electronically to HFLLicenseAction@mass.gov, for a period of 14 days following this publication. All written or oral comments submitted to the Department may be posted on the Department’s website and released in response to a request for public records.
