Priority violations:
• Spray bottle of bleach water is not registering any concentration on the strip. Nonpriority violations:
• Labels on uncooked chicken are out of alignment so the words uncooked chicken are not real legible.
• Chicken or other products that have more than one ingredient (i.e. breaded) must have the ingredients listed on label.
• Condensation leak in freezer.

Priority violations:
• Cooked items in the cooling process still cooling (within the correct time) but ensure: Cool from 135°F to 70°F in 2 hours or less. From 70°F to 41°F or below in 4 hours or less.
Nonpriority violations:
• Storing unwrapped sausage in T-shirt-type bags.
• Area behind fryer and under table (shelf) is not clean.
• Dishes stacked directly on cloth towel to drain. Dishes should not come in contact with absorbent material. Use something that is non absorbent or nothing at all.

Priority violations: • Cut lettuce sitting out at room temperature. (Put in cooler.)
• Two-drawer unit is still not holding food at 41°F or below. (Moved to other cooler.) Nonpriority violations:
• Still no thermometer in drawer unit.
• Sinks not draining (three-compartment).
• Need plugs for sinks.

Priority violations:
• Handwashing sink has a bucket in it. Hand sink must be accessible at all times. Corrected.
• Milk 51°F in small refrigerator. Refrigerator thermometer reading 50°F. Adjusted. Ensure milk is held at 41°F or below. Nonpriority violations: • Need new plugs for three-compartment sink.
Cathy’s Corner, 18446 Highway 412
Priority violations:
• Employee drinks on preparation table and one on slicer. Corrected.
• Employee did not wash hands after coming in from outside and before putting on gloves. Corrected.
• Employee did not wash hands between handling dirty dishes and clean dishes. Corrected.
• Pan of gravy in the hotholding unit at 87°F. Item was not reheated properly prior to placing in hot holding. Reheat to 165°F in 2 hours or less prior to placing in hot holding. Item reheated in the stove.
• Items in the children’s section of the menu cannot be offered under-cooked (hamburger, eggs). Items on children’s menu should not be marked with an asterisk (*). Correct next menu change.
• Sanitizer concentration dispensed at the dish machine exceeds the allowable 100 ppm concentration. Adjust as needed.
Nonpriority violations:
• Some plastic containers cracked, broken or the plastic is bubbling. Replace containers as needed.
• Shelves in salad cooler are rusty. Replace or recoat using an approved method. Repeat violation.
• Bottom of inside freezers in outside storage are not clean. Come up with a cleaning schedule for units. Complete by 1/13/17.
• Bottom of inside salad cooler is not clean. Clean at the end of shift.
• Floor in kitchen is no longer in good condition, tile is peeling in several places.
• Light in walk-in is not shielded or shatter resistant.
• Permit posted in the office area and is not in customer view.

Priority violations:
• Dish washer did not wash hands between handling dirty dishes and clean dishes. Corrected.
• Beans are 123°F on the steam table. Reheat foods to 165°F before placing on steam table. Corrected.
• Food workers not wearing effective hair restraints.
• Bottom inside of both freezers are not clean.

Nonpriority violations:
• Lettuce stored in back cooler uncovered.
• Scoop at one beverage station is stored in a broken Styrofoam cup. Store scoop on or in something that can be cleaned and sanitized. Repeat violation.
• Food spilled on the floor in the walk-in.
• Cabinets under both soda machines/ice bins are not clean.
• Gasket torn on twodoor upright cooler in servers area.
• Light above grill is missing the cover. Replace as needed.

Nonpriority violations:
• Test strips are no longer valid. Looks like they got wet. Facility has another container of strips.
• Silicone around mop sink is coming out. Replace as needed.
• Missing handle on lid of one cooker. Replace handle as needed.
• Tile broken in front of the fryer. Replace tile or patch as needed.
