Sweet Water Tavern, 201 W. University St. Priority violations: • Person in charge did not know which sanitizer test strips were for which sanitizer chemical or the correct concentrat­ion for the different sanitizers. Manager was trained on this aspect during inspection.

• Employee did not wash hands after handling raw meat products/ containers before putting on gloves for a task change. Corrected.

• Employees washing hands in a sink that was designated as a mop/service sink. Hands should be washed in a designated handwashin­g sink only. Repeat violation.

• Mashed potatoes on steam table were 119°F. Corrected, item reheated as needed.

• Sausage stored above the load line. Top of sausage is 44°F. Item destroyed as needed. Nonpriorit­y violations: • Employees must wear effective hair restraints.

Allen Elementary, 1900 N. Mt. Olive Nonpriorit­y violations: • Metal rack used for drying rack is rusting and no longer in good condition. Replace or recoat.

• Clean fan and fan guards in walk-in cooler.

• Condensati­on leak in freezer. Do not store open boxes or packages under area that is dripping. Repeat violation.

• Crack in floor in front of threshold of walk-in freezer. Seal crack to prevent further damage to floor. Repair prior to fall semester 2017.

Siloam Springs Intermedia­te School, 1500 N. Mt. Olive Priority violations: • Diced ham in walk-in is at 44°F. Item may have been out for portioning. Placed in freezer to bring down temperatur­e rapidly. Product was down to temperatur­e by end of inspection. Nonpriorit­y violations: • Ice build-up on freezer door which is causing the door gasket to not seal properly. Door does not close all the way with ice build-up. (A new gasket was installed recently.) Manager removed ice from door frame so door closes all the way.

Station III, 1295 N. Mt. Olive St. Priority violations: • Pan of raw meat stored on top of a container of Canadian bacon. Corrected.

• Spray bottle not labeled with contents next to pizza oven.

• Spray bottle of cleaner with bleach stored on a food preparatio­n area. Corrected.

Nonpriorit­y violations:

• Employee beverage sitting on the preparatio­n unit. Corrected.

• Broom handle and cases of water in front of handwashin­g sink, making it no longer easily accessible.

• Interior of cappuccino machine is not clean.

• Ice scoop stored on a towel on the ice machine. Store scoop on or in something that can be washed and sanitized regularly.

• Part of the ceiling is missing from the walk-in cooler. Manager said the unit is going through a remodel.

Quality Inn, 1300 Hwy. 412 W. Priority violations: • Sanitizer is a different brand than what is labeled on the bottle. Do not use other chemical bottles for different brands of chemicals even if they seem to be the same product. Contact the company to set premix unit for the new brand of chemical.

• Premix sanitizer is too strong. Contact company to adjust unit. Nonpriorit­y violations: • Permit posted but expired.

Super 8 Motel, Hwy. 412 W. Priority violations: • Hot water knob missing from handwashin­g sink. Replace. 1800
