Asian Chef, 315 C Highway 412 West Priority violations:

• Lettuce for salad is held on ice but the level of the lettuce is above the ice level and is not being held at 41°F or below. For ice to be effective, the ice must be at or above the level of the food.

• Cooked items or other potentiall­y hazardous foods not used in 24 hours must have a date mark. For example but not limited to: fried chicken (used for sweet and sour), egg rolls and spring rolls, open packages of tofu. Repeat violation. Non-priority violations:

• Some food stored uncovered in the walk-in. Shelves above items are not clean. Debris from shelves can fall into food. Repeat violation.

• Reusing a soy sauce bucket for mixing corn starch. Single use containers cannot be reused for food storage.

• Cardboard being used to cover the shelves above the prep table and other table. Cardboard is not an approved material for food areas. Remove cardboard and clean shelves.

• Cardboard wrapped in plastic wrap is being used to separate blocks of chicken. Cardboard is not to be used even if covered. Use something that can be cleaned and sanitized and does not have to be cut to fit unless the edges can be smooth and easily cleanable.

• Door between kitchen and dining room has an accumulati­on of dirt/ grease. Clean door as needed and repaint or cover the area that is no longer cleanable.

• Shelves in walk-in cooler have an accumulati­on of food and dirt. Clean regularly to prevent the accumulati­on of dirt.

• Center drain in threecompa­rtment sink is leaking.

• Unused/broken restrooms in the back area must be either maintained in a clean manner or cap off the toilets. Strong sewer/urine smell in the back room by storage shelves.

• Remove unnecessar­y items from back storerooms to help keep facility clean. This is an on-going repeat violation.

• Employee did not lather long enough before rinsing off soap. Lather for 10-15 seconds then rinse off soap. Corrected.

• Food items are not date marked as required. Potentiall­y hazardous food not used in 24 hours must have a date mark. Repeat violation. Non-priority violations:

• Foods being thawed at room temperatur­e. Items will be put in oven for proper cooking.

• Food workers shall wear hair restraints, beard restraints and clothing that covers body hair.

• Except for a plain ring such as a wedding band, while preparing food, food workers may not wear jewelry on their arms and hands.
