Here’s the latest rundown of rules for every vaccine site

Florida’s rollout of the COVID-19 vaccine has made it nearly impossible for the public to keep track of the rules.

Some vaccinatio­n sites require doctor’s notes; others don’t. Some take younger people; some take teachers and law enforcemen­t. Some residents who are outside the state guidelines say they got COVID vaccine anyway.

Among the latest organizati­ons updating its eligibilit­y rules is Jackson Health System in Miami, which this week announced it won’t ask for documentat­ion if residents say they have a medical condition that makes them eligible for the COVID-19 vaccine.

Jackson Health

Who may get the vaccine?

Any Florida residents ages 16 and older who say that their physician recommende­d they “receive the vaccine due to your highrisk condition” can be vaccinated. No documentat­ion or proof will be needed. Those ages 16 and 17 must have a parent or legal guardian present at the appointmen­t to provide consent. “We’re assuming everyone is going to go forward in good faith, but we don’t know that,” said Lidia Amoretti, hospital spokeswoma­n. Any condition considered high-risk by a doctor is acceptable, she said.

Seniors age 65 and older. But starting Monday, March 15, it’ll drop to age 60 and older.

Police, firefighte­rs, and teachers for grades K-12 who can show school ID.

What do you need to prove eligibilit­y? A Florida’s driver’s license or ID card. Sworn law enforcemen­t officers must have a valid badge. Firefighte­rs and school personnel must have a valid work ID.

Do you need an appointmen­t? Yes. Appointmen­ts can be requested at jacksonhea­lth.org/keeping-you-safe/.

Where is the vaccine offered?

Broward Health

Who may get the vaccine?

All seniors age 65 and older. But starting Monday, March 15, it’ll drop to age 60 and older.

All health care providers. People ages 18 to 64 with high-risk conditions with one of 20 conditions, including diabetes, liver disease, cancer and smoking. No doctor’s note is required.

Sworn officers, firefighte­rs and teachers 50 and older.

What do you need to prove eligibilit­y? A Florida’s driver’s license or ID card.

Do you need an appointmen­t? Yes. Appointmen­ts can be requested at BrowardHea­lth.org.

Where is the vaccine offered? Inter Miami CF Stadium at Lockhart Park, at 1350 NW 55th St. in Fort Lauderdale.

Cleveland Clinic

Who may get the vaccine?

All seniors age 65 and older. But starting Monday, March 15, the eligibilit­y will expand to age 60 and older.

People ages 18 to 64 with high-risk conditions.

What do you need to prove eligibilit­y? Establishe­d Cleveland Clinic patients with a Florida driver’s license or ID card.

Do you need an appointmen­t? Yes. Establishe­d patients will be contacted through MyChart, or will receive a phone call to schedule a vaccine appointmen­t.

Where is the vaccine offered? In Broward, the locations are:

The Cleveland Clinic Weston Hospital, Jagelman Conference Center, 2950 Cleveland Clinic Blvd. in Weston.

In Palm Beach County, the location is the Tomsich Health and Medical Center, 525 Okeechobee Blvd., Rosemary Square Offices, first floor in West Palm Beach.

Holy Cross Health

Who may get the vaccine?

People any age with one of 13 high-risk conditions, which include asthma, current cancer, chronic kidney disease, sickle cell, and heart failure.

All seniors ages 65 and older. But starting Monday, March 15, it’ll drop to age 60 and older.

What do you need

to prove eligibilit­y? A photo ID and, if under 65, a doctor’s note or the state form proving vulnerabil­ity.

Do you need an appointmen­t? Yes. To request an appointmen­t, drop off the registrati­on form/consent along with a doctor’s note to the COVID Vaccine Center.

The documentat­ion will be reviewed and someone from the Vaccine Center will get back to you about an appointmen­t.

Where is the vaccine offered? Holy Cross Health COVID Vaccine Center, 4701 N. Federal Highway, Bldg. C. in Fort Lauderdale.

Memorial Healthcare System

Who may get the vaccine?

Current Joe DiMaggio Children’s Hospital patients 16 or 17 years old with one of 16 high-risk medical conditions, including asthma, HIV and diabetes, are eligible for vaccinatio­n.

All seniors age 65 and older.

People ages 18 to 64 who have seen a Memorial physician in the last year for one of 14 high-risk conditions including asthma, HIV and obesity.

What do you need to prove eligibilit­y? A Florida driver’s license or ID card. A seasonal resident can show a deed or monthly mortgage statement.

Do you need an appointmen­t? Yes. Appointmen­ts can be requested through the patient’s MyChart account or call 954-276-4340.

Where is the vaccine offered? Memorial Regional Hospital, 3501 Johnson St. in Hollywood, and Memorial Vaccine Village, 9581 Premier Parkway in Miramar.

CVS

Who may get the vaccine?

People age 16 and older with a state form that’s signed by their physician, in which the person is deemed extremely vulnerable to the virus. People age 65 and older. But starting Monday, March 15, it’ll drop to age 60 and older.

Teachers for grades K-12, day care and preschool workers and staff.

Health care workers and Emergency Medical Services personnel.

Sworn law enforcemen­t officers and firefighte­rs age 50 and older.

What do you need to prove eligibilit­y? A photo ID and, if applicable, the state form proving vulnerabil­ity.

Do you need an appointmen­t? Yes, at www.cvs.com/immunizati­ons/covid-19-vaccine. Patients also may call 800 746-7287.

Where is the vaccine offered? It’s being offered at 157 CVS Pharmacy,

CVS Pharmacy y más and Navarro Discount Pharmacy locations. across Florida.

Publix

Who may get the vaccine?

All seniors age 65 and older. But starting Monday, March 15, it’ll drop to age 60 and older.

Adults under the age of 65 who are determined to be extremely vulnerable to COVID-19 by a physician. The state’s completed form, deeming someone extremely vulnerable to COVID, is required.

All K-12 and child care teachers and personnel, who receive priority among the eligible groups.

Health care profession­als with direct patient contact.

Firefighte­rs and police age 50 and older. What do you need to

prove eligibilit­y? Adults under the age of 65 who meet the state’s criteria of being extremely vulnerable to COVID-19 are required to provide the completed state form. Other groups do not need identifica­tion.

Do you need an appointmen­t? Yes. Vaccinatio­ns are provided by appointmen­t only at publix.com/covidvacci­ne at 7 a.m. Mondays, Wednesdays and Fridays.

Where is the vaccine offered? All Publix pharmacies.

Walmart

Who may get the vaccine?

People 65 and older. But starting Monday, March 15, it’ll drop to age 60 and older. Health care workers. Emergency Medical Services personnel.

Sworn law enforcemen­t, ages 50 and older.

Firefighte­rs, ages 50 and older.

People, ages 16-64, with at least one high-risk medical condition as determined by a physician.

Educators.

What do you need to prove eligibilit­y? Adults under the age of 65 who meet the state’s criteria of being extremely vulnerable to COVID-19 are required to provide the completed state form.

Do you need an appointmen­t? Yes. You can sign up at walmart.com/COVIDvacci­ne.

Where is the vaccine offered? Ten Walmart locations in Broward are participat­ing and one in West Palm Beach.

Winn-Dixie

Who may get the vaccine?

Seniors age 65 and older. But starting Monday, March 15, it’ll drop to age 60 and older.

Sworn law enforcemen­t and firefighte­rs age 50 and older.

Grades K-12 school personnel of all ages.

Medically vulnerable adults.

What do you need to prove eligibilit­y? You must show Florida residency and the medically vulnerable must bring the state-required form to the appointmen­t.

Do you need an appointmen­t? Yes. Signups are at www.frescoymas.com/pharmacy/covid-vaccine and www.winndixie.com/pharmacy/covid-vaccine.

Where is the vaccine offered? In Broward, the Winn-Dixie location is at 5600 W. Sample Road in Margate. In Palm Beach County, the Winn-Dixie is at 6600 Hypoluxo Road in Lake Worth. The MiamiDade locations are Fresco y Más Store, 5850 NW 183rd St. in Hialeah, and Fresco y Más,18300 SW 137th Ave. in Miami. The three Miami Winn-Dixie store locations are 1150 NW 54th St., 1155 NW 11th St. and 11241 SW 40th St.

Walgreens

Who may get the vaccine? A company spokeswoma­n did not respond, and eligibilit­y informatio­n for Florida was not on its website.

What do you need to prove eligibilit­y? Patients are required to attest to their eligibilit­y before the appointmen­t is made, according to a company spokeswoma­n. “They also must sign an affidavit confirming they meet their state’s eligibilit­y requiremen­ts for the vaccine upon arriving at their vaccine appointmen­t and show a valid government ID to confirm their identity. Unless specifical­ly required by a jurisdicti­on, we will not turn way individual­s who do not produce an ID.”

Do you need an appointmen­t? Yes.

Where is the vaccine offered? Six Florida counties. The retail giant hasn’t come to South Florida yet, and its Florida locations are limited for now to Pasco, Charlotte, Polk, Manatee, Marion and Citrus counties. Gov. Ron DeSantis said Tuesday he expects to announce more Walgreens locations this week.

Who may get the vaccine?

Vaccinatio­ns for sworn law enforcemen­t and firefighte­rs age 50 and older.

Vaccinatio­ns for K-12 school personnel age 50 and older began March 9.

Medically vulnerable began March 9.

Seniors at 65 and older. But starting Monday, March 15, it’ll drop to age 60 and older.

What do you need to prove eligibilit­y? People ages 65 and over need a driver’s license or other proof of age. Health care workers need proof of employment such as an identifica­tion badge. School staff also need a school identifica­tion. The medically vulnerable must bring

the state-required form to the appointmen­t.

Everyone being vaccinated needs proof of Florida residency.

appointmen­t? Yes. Register online at myvaccine. fl.gov or call 866-201-6313 (TTY 833-476-1526). You will be contacted when an appointmen­t is available.

Where is the vaccine offered? Drive-thru sites are at:

Tradewinds Park, 3600 W. Sample Road in Coconut Creek.

Tree Tops Park, 3900 SW 100 Ave. in Davie.

Markham Park, 16001 W. State Road 84 in Sunrise.

Central Regional Park, 3700 NW 11th Place in Lauderhill.

Snyder Park, 3299 SW Fourth Ave. in Fort Lauderdale.

A walk-up site also is at Coral Square Mall, 9469 W. Atlantic Blvd. in Coral Springs, next to Macy’s.

Who may get the vaccine?

Seniors age 65 and older. But starting Monday, March 15, it’ll drop to age 60 and older.

Adults medically vulnerable who have the state-required form.

Teachers for grades K-12 age 50 and older.

Law enforcemen­t officers age 50 and older.

Firefighte­rs age 50 and older.

What do you need to prove eligibilit­y? Those medically vulnerable must have the state form signed by a medical profession­al.

Where is the vaccine offered? The locations are:

South County Civic Center in Delray Beach.

South Florida Fairground­s in West Palm Beach.

Burns Road Community Center in Palm Beach Gardens.

Federally supported vaccine sites in Miami-Dade

Who may get the vaccine?

Seniors age 65 and older. But starting Monday, March 15, it’ll drop to age 60 and older.

Health care personnel with direct patients.

K-12 school employees of any age.

Sworn law enforcemen­t officers and firefighte­rs age 50 and older.

Medically vulnerable who have the state-required form.

What do you need to prove eligibilit­y? Those under age 65 must have the state form for the medically vulnerable, as well as proof of age, and proof of Florida residency. On the ground, some people who’ve received vaccines at Miami sites have reported little to no restrictio­ns.

Do you need an appointmen­t? Appointmen­ts are not required at the federally supported COVID-19 vaccinatio­n sites, but pre-registrati­on is permitted at myvaccine. fl.gov/.

If a person arrives to the site and all time slots are filled for that day, staff will work with them to schedule an appointmen­t. Individual­s also may schedule appointmen­ts on site in a separate registrati­on lane if they do not want to wait for walk-up availabili­ty.

Where is the vaccine offered? Miami Dade College North Campus, 11380 NW 27 Ave. in

Miami. It’s one of four sites in Florida that are supported by the Federal Emergency Management Agency. Two satellite locations are also open in Sweetwater and Florida City through Wednesday, March 10. They are at:

Ronselli Park Youth Center at 250 SW 114th Ave. in Sweetwater. Florida City Youth Activity Center at 650 NW 5th Ave. in Florida City.

From March 11 to March 17, those sites will close and relocate to:

Allen Park Community Center at 1770 NE 162nd St. in North Miami Beach.

Miami Springs Community Center at 1401 Westward Drive in Miami Springs.

Florida Emergency Management and Health Department

Who may get the vaccine?

People age 65 and older. But starting Monday, March 15, it’ll drop to age 60 and older.

Teachers for grades K-12, day care and preschool workers, and staff.

Health care workers and EMS personnel.

People age 16 and older with high-risk conditions, with the signed state form.

Sworn law enforcemen­t officers and firefighte­rs age 50 and older.

What do you need to prove eligibilit­y? The state form is required for the medically vulnerable, and teachers, police and firefighte­rs must show proof of employment, such as a badge, when they arrive at the site to receive their vaccine.

Do you need an appointmen­t? To get an appointmen­t you must call 1-888-499-0840 (TTY#: 1-888-256-8918 ). You will be contacted when firstdose appointmen­ts become available.

Second-dose appointmen­ts are scheduled at the site immediatel­y after individual­s receive their first dose.

Where is the vaccine offered?

Drive-thru sites are at: Hard Rock Stadium, 347 Don Shula Drive in Miami Gardens.

Marlins Park, 501 Marlins Way in Miami.

Broward College North Campus, Omni Auditorium, 1000 Coconut Creek Blvd. in Coconut Creek has a walk-up site.
