The report indicates that Crawford was suspected of being under the influence of alcohol. A blood-alcohol test was conducted, the report said, but the results were unknown.

Cameron Keir, a Dublin spokesman who handles police department communicat­ion, said charges were pending against Crawford but he could not say what they would be. A check of Delaware County Municipal Court records didn’t immediatel­y turn up anything. The Dublin records indicated that local attorney Larry Reihl arrived at the hospital lobby to meet Crawford after the crash. A message left for Reihl was not returned Tuesday.

Public records matching Crawford’s age and address indicate he holds a medical license in Ohio and is a former Army doctor and current surgeon at a practice in New Albany.

The reports released Tuesday also name the fourth person in the car as Thomas Alan Nichols, 24, of Granville.

According to the crash report, Crawford was driving a 2014 BMW north in the 5000 block of Muirfield Court when the car went off the right side of the road and hit a culvert before going airborne into a tree. The Dispatch reported Sunday that Wisniewski and Cauley were both injured in the crash, which occurred just after 11 p.m. not far from the golf club where the second round of the Memorial Tournament had ended earlier that day. Both were hospitaliz­ed.

Top-ranked golfer Justin Thomas is a friend of Cauley’s and told The Dispatch over the weekend that Cauley had surgery after the crash left him with a collapsed lung, five broken ribs and a fractured lower left leg. Cauley’s handlers later confirmed those injuries. Wisniewski, Thomas said, had broken some ribs.

Thomas said he had been staying at Wisniewski’s Dublin home during the tournament. Wisniewski, who played four seasons (2011-15) for the Blue Jackets, played for the United States at the Winter Olympics in February.

In an Instagram post on Monday morning, Cauley said the past few days have been tough and he thanked everyone for their support, well-wishes and encouragem­ent.

“Friday was the scariest night of my life but I’m so thankful to be alive and only have the injuries I have,” he wrote. He said he has no timetable for his return to profession­al golf but that surgery on Sunday was successful.

“Every day is a step closer to competing again,” Cauley wrote. “In the meantime, I’ll use this time to reflect and get stronger.”

Wisniewski’s health status wasn’t available Tuesday. The nature of Crawford’s and Nichols’ injuries remain unclear.

The 911 calls indicate that the on-board system for the BMW initiated one call to report the crash and another came from a neighbor. That man told the dispatcher that the four people in the car “need help right away.”
