WHO: Isabella AGENCY: Reliance Health Inc. HER SITUATION: Isabella is a 30-year-old mother with diagnoses of bipolar disorder, post-traumatic stress disorder and multiple sclerosis. She has four children ages 12, 8, 3 and 1. Isabella had to start over with her family, as her apartment had been condemned, and she had to immediatel­y move out and leave most of her belongings. Isabella's primary goal is to focus on her kids and have them feel comfortabl­e and happy in their new apartment. She always puts her kids' needs first, even with her painful physical challenges. Her dream is to take all of her kids on a family bike ride. WHAT TO GIVE: Mom: Large jacket, socks, leggings

Daughter (12): Bicycle, bedding set (twin), hats, gloves, socks

Son (8): Dinosaur and sharks, bedding set (twin), hats, gloves, socks

Daughter (3): Size 3T-4T clothes, pull-ups, toddler toys (Minnie Mouse), a bedding set (twin)

Daughter (1): Crib or pack and play, diapers (4), clothing (12-18 months), baby wipes, bottles and Similac sensitive formula

Family: Food donations, Christmas decoration­s, computer, cleaning supplies, hygiene products, Tupperware, DVD player (with kids movies) and picture frames
