Q: I’m Missing 4 Teeth. Do I Need 4 Implants?

A: I have this discussion frequently whenever I see multiple teeth missing. Just recently, I met a woman who had some implants in the back of her mouth. A dentist placed them years ago, and she was doing well with those teeth. She came to me for an evaluation of several loose teeth in the front of her mouth. A wire held these teeth in her mouth as their looseness would result in teeth falling out.

I suggested that we remove five teeth and use implants to replace these teeth. My patient was excited to learn that I did not need five implants to replace these teeth. I could utilize as little as two or three implants for replacing the missing teeth. I employ a technique called an implant bridge to fill the gaps of the missing teeth. She saved money as the costs go down, and the treatment is easier for all. Another factor also plays into this scenario. When implants are not right next to each other, the ability to create beautiful cosmetics is no longer a challenge. Space is a significan­t asset and a friend to the dentist and the dental laboratory. Adequate room in all directions allows greater freedom with the shape and position of the tooth. The gums can look natural and the way that the teeth meet the pink gums is enhanced. All of these little details lead to a more natural and comfortabl­e result.

I can give a patient ten teeth on just four or five implants. Every case is different, and many factors determine the number of implants. However, this 2:1 rule is a good starting point. Details of the sequence of treatment, temporary cosmetic replacemen­t and healing time all come into play when I outline a personaliz­ed plan for you. My patient with the loose front teeth found that I could place implants on the same day that I removed her teeth. Her experience was positive and she left the office with an attractive smile. When she came to me, her front teeth were so loose that simple chewing was a challenge for her. This transforma­tion was a marked improvemen­t in less than an hour.

When I discussed treatment with her, she asked me what treatment I would provide to my wife if she were in the chair. I had no qualms or hesitation telling her that the plan for my wife was the same one that I just presented to her. Strategica­lly placed implants allowing the maximum number of teeth as required by the patient is the approach that I like to follow.
