BCI offering free physicals for Bremen students

For the fourth year, Bremen Castings, Inc. (BCI) is offering free physical examinations to student-athletes attending Bremen Public Schools. Examinations will be conducted on Tuesday, June 22 from 9 a.m. to 7 p.m. at BCI’S Activate Health and Wellness Center located at 710 W. Dewey Street. Physicals will be performed by a certified nurse practitioner. Results from the physical examination will be recorded on the IHSAA Pre-participation Evaluation form.

Every Bremen student-athlete must have an annual physical examination to participate in school sponsored athletics during the 2021-22 school year, so take advantage of this gen

erous offer for your student-athlete!

In order to receive a physical please follow the steps below:

• Print the athletic physical form (physical), complete pages 1,2,5 and take the the entire physical form (pages 1-5) to the health center.

• Remain in your car when arriving at the BCI’S Activate Health and Wellness Center and call 574-248-4856.

• BCI will perform a COVID screen prior to entering the clinic and receiving the exam.

• Once completed, please show your appreciation to BCI for their free service to the Bremen community.

• After the exam and receiving the completed physical form, upload document at Register My Athlete or drop off the form at the Bremen athletic office or the central office building (across from high school).
