HHC now offers Continuous Care

Hospice Home Care now offers the most difficult and underutili­zed level of care under the Medicare Federal Hospice Benefit – Continuous Care. Not all hospice companies provide this service to patients and their families.

This one-on-one private, in-home care is provided to hospice patients when symptoms develop that require around-the-clock care from highly trained profession­als. This type of in-home continuous care is typically short-term and referred to as “crisis care.” This is a difficult level of care to initiate and few hospices elect to be nimble and willing to provide this service to patients and their families, a news release said.

The Centers for Medicare and Medicaid Services statistics say that less than half of 1 percent of hospice patient days are Continuous Care. Hospice Home Care believes that agencies willing to do the tough services do all services better. Being faithful and willing to do the difficult care makes us better at all care.

A short-term continuous care episode with Hospice Home Care is physician directed and includes 24-hour attention by a registered nurse, licensed practical nurse and aide.
