Non-closure inspections of note

-

Here are the food facilities that inspectors temporarily shut down because of imminent health hazards between Feb. 26 and March 4 in Riverside and San Bernardino counties. If no reopening date is mentioned, the facility had not been listed as reopened as of Friday, March 5.

SUSHI ZEN, 1525 E. ONTARIO AVE. SUITE 108, CORONA Closed: Grade: Reason:

Monday and Wednesday 70/C, failing

No hot water. The facility didn’t have hot water when the inspector arrived March 1 so it had to close briefly until the water heater was turned back on. Among other violations, several employees didn’t use soap to wash hands, including one who cleaned out a floor sink and another who handled raw fish; dishes with food debris and mold growth had been put away with clean dishes; seafood, cooked vegetables and rice weren’t being kept at safe temperatures; tuna was being thawed unsafely; and surfaces throughout the kitchen were dirty. This was the third consecutive routine inspection that the restaurant failed, and it now faces a permit suspension/revocation hearing. When an inspector returned March 3 to deliver paperwork, the hot water was out again so the restaurant was temporarily closed.

84/B, failing

No sanitizer. A food facility may not operate without sanitizer to clean dishes and other surfaces so it had to close briefly until the manager could get some bleach. Among other violations, many of which the inspector noted were repeated issues, sinks in the kitchen and women’s restroom didn’t have soap; the floors, walls, a freezer, a refrigerator and the yogurt machine nozzles were dirty; and the person in charge didn’t know how cold to keep food or how to wash dishes properly. The restaurant failed an inspection and was shut down for not having sanitizer for a day in October, so it now faces an administrative hearing.

Cockroach infestation. The inspector visited in response to a complaint that the restaurant was dirty and someone saw a cockroach. The owner was aware of the infestation and was working with pest control — the inspector found “several sticky pads full of live and dead roaches” and a few live roaches in other areas — and while the facility was being cleaned at the time, it hadn’t shut down. The inspector did that immediately. There were also rodent droppings under a storage rack.

Cockroach infestation. The inspector found live roaches on a cooler, under a food-prep table and near the water heater and mop sink, dead roaches on the floor and a wall, and roach droppings on the same cooler and a wall. Among other violations, there was a can of Raid (not approved for use in restaurants) and the inspector noted “an accumulation of grime, grease and old food debris” on all of the cook-line equipment, coolers, dry storage racks, hood filters, light switches, walls, floors and ceiling.

Rodent infestation. The inspector visited in response to a complaint about rodents. The manager was aware of the infestation and was working with pest control but hadn’t closed. The inspector found droppings on and under shelves containing food and cooking utensils for sale, and throughout a storage area. Several bags of candy and snacks had been chewed open. The inspector returned March 3 and 4 but the infestation hadn’t been eliminated.

Fire damage. There had been a grease fire. The facility was being cleaned but the hood still needed servicing. The inspector noted that the hood filters weren’t installed correctly.

No hot water. In the Feb. 23 inspection, the handwashing and dishwashing sinks didn’t have hot water. The inspector went back Feb. 25 and found the water to not be quite hot enough in the whole facility. The restaurant was given 24 hours to fix it. The next day, the water heater was being replaced and there was no hot water at all but the restaurant was still open and taking orders, so the inspector shut it down.

Later Feb. 26 after the water heater was up and running

Here are selected inspections at facilities that weren’t closed but had other significant issues.

BurgerIM, at 13394 Limonite Ave. Suite 110 in Eastvale, was inspected March 3 in response to an unconfirmed foodborne illness complaint. It received a failing grade of 82/B with one critical violation: The inspector described the facility as “in severe need for cleaning” in numerous areas including food-contact surfaces. Among the 10 lesser violations, an employee didn’t wash hands after touching raw meat, two kitchen sinks didn’t have soap and one didn’t have hot water.

Pie for the People! Pizza De Circo, at 58960 Twentynine Palms Highway in Yucca Valley, was inspected March 1 and received a grade of 83/B with two critical violations. An employee didn’t wash hands, and some marinara sauce was not being kept hot enough. Among the five lesser violations, employees weren’t properly keeping track of how long cooked pizzas were left out at room temperature, raw chicken wings were being thawed unsafely and the facility was using a refrigerated transport trailer parked outside as a walk-in refrigerator, which it was told to discontinue.
